How do cardiac care nurses help with the physical and emotional challenges of recovery? "We monitor a patient’s heart activity, administer medication, perform stress test evaluations and generally help them get back to the same, or better, level of function than they had before their heart attack or before they needed surgery," explains Wesley Shipes, RN. "Many of us have extra certification in critical care or in treating cardiovascular surgery patients. You need to understand the cardiovascular system inside and out to be able to help treat the patients here. "It’s a team approach to get patients hemodynamically stable, meaning their heart is pumping well and they have good circulation, at the bedside. Of course we work with cardiologists, but we also work with cardiothoracic surgeons, respiratory therapists, and the cardiac rehab unit. One of the major tasks before discharge is education for patients about taking care of themselves so that they don’t end up back in the hospital." UM surgeon at center of settlement says he does 3 to 5 surgeries a day. Is that possible? “Dr. Lamelas has performed more than 16,000 cardiac surgeries throughout his 28-year career, 7,000 of which have employed a minimally invasive approach,” the second paragraph of UM’s announcement read on Jan. 2, 2019. And if you divide 16,000 surgeries by 28 years, the math shows that Dr. Lamelas would have performed 571 surgeries in a given year, or roughly 1.5 surgeries every day, seven days a week Puffery comes in two varieties, one that stretches the truth a little with no real harm and the other that is potentially dangerous and harmful. Calling all clinicians (Medics, GPs, Ophthalmologists, Nurses, Optometrists, Orthoptists and Pharmacists) who triage acute eye presentations within primary, secondary and tertiary care. I am pleased to report our paper identifying key symptoms that could support current red flags to triage patients for emergency ophthalmic examination and elective care (GP/optometrist examination) from over 12,000 emergency attendances Moorfields Eye Hospital, London covering almost 100 ophthalmic conditions has been published in Eye (Nature). Summary What was known before - Demand for emergency-based eye services within the hospital setting is continuing to increase. - A significant proportion of those attending can be managed safely outside the emergency care setting by appropriately trained non-medical healthcare professionals. - Effective history-taking is essential for safe and effective triage for patients to attend the appropriate setting. The Royal College of Ophthalmologists has recommended further upskilling and support of healthcare professionals but this could require significant resources to enable this. What this study adds - A multivariate analysis from over 12,000 patient attendances from an acute tertiary care setting covering almost 100 conditions found twelve statistically significant features that could support clinicians to triage patients to emergency same-day and could supplement current red flags as well as ten statistically significant features for elective care. -These features could be rapidly integrated into existing triage pathways carried out by healthcare professionals that could direct patients to the appropriate setting and support their decision-making that may reduce same-day attendances to hospital emergency departments.
