It ensures that the project deliverables are effectively integrated into current practices, that stakeholders are prepared for the changes, and that any disruptions are minimized. Some key steps are : 1 Planning the Transition: It involves; a) Define Objectives: Clearly outline what needs to be achieved during transition. b) Develop a Transition Plan: Creating a detailed plan that includes activities, timelines, roles, responsibilities, and resources required as well as appointing a Transition Lead. 2. Communication: This involves; a) Stakeholder Engagement: Identify and engage all key stakeholders early in the transition process to ensure their support and address any concerns and as well appoint a Transition Lead for your project. b) Communication Plan: Develop and implement a communication plan to keep stakeholders informed and provide updates on the progress of the transition. 3. Training and Support: a) Training Programs: Provide training sessions for end users to ensure they are familiar with new systems, processes, or tools. 4. Implementation: a) Feasibility Study: Conduct feasibility study to identify potential issues before full-scale implementation. 5.
