Formal strategic planning began in May, 2017 with assistance from a private consulting firm to make recommendations about structure, governance, member services, policy and program priorities, etc…

    A national coordinating committee (NCC) of 20 CHW leaders and allies from around the country has been meeting regularly for over a year to make plans for a building a sustainable membership organization for CHWs—broadly defined to include CHWs, CHRs, promotores, other workforce members—and allies. Formal strategic planning began in May, 2017 with assistance from a private consulting firm to make recommendations about structure, governance, member services, policy and program priorities, etc. The NCC recently conducted a national survey of state, regional, and local CHW associations to assess the current capacity of workforce networks and to enable broad participation in planning. In July, 2017, the NCC agreed to serve as an interim board for the National Association of Community Health Workers, with the goal of electing a permanent board, securing resources, incorporating, and launching the organization at 2019’s Unity Conference in Las Vegas, Nevada.

To begin a larger community‐based participatory study, the purpose of this study was to explore community residents’ perceptions of barriers and facilitators to achieving and maintaining health.

    Objective
    Health inequities and disparities are associated with non‐White race/ethnicity, immigrant status, income, and geographic location. Community engagement is essential to identify health and social needs and to plan health care and social services programs. To begin a larger community‐based participatory study, the purpose of this study was to explore community residents’ perceptions of barriers and facilitators to achieving and maintaining health.

    Design, Sample, and Measurements
    This qualitative descriptive study used focus group interviews. We recruited a convenience sample (n = 50) from community meetings and gathering for five audio‐recorded focus groups that used a semi‐structured interview guide. Transcripts were coded to identify common topics in each group and major themes across groups.

    Results
    Participants were predominantly women (58%), Hispanic/Latinx, and Spanish‐speaking (57%), who rented their homes (69%). Two main themes emerged: (a) social determinants as barriers to health and (b) need for trust to participate in health programs.

    Conclusions
    Although health care providers are frequently concerned about providing access to care, community members identified a variety of social determinants that affected their health. Listening and responding to community members’ priorities are the foundation to improving health in neighborhoods directly affected by inequities.

    The goal of the project is to develop a platform that will connect clinical and social services data in order to close the loop for social-services referrals between health clinics and social care organizations in the community. The platform will use data from the Austin-based non-profit organization Integrated Care Collaboration (ICC), which functions as the area’s health information exchange (HIE), and Aunt Bertha, a social services referral digital database and application. The platform will allow health care providers, to see relevant information related to social determinants of health and social referrals in a user-friendly dashboard alongside their electronic medical records (EMR) with minimal clinical workflow
    disruption.

    The data platform, called Social Services and Health Information Platform (SHIP), is part of a larger
    “model community” effort that includes coordination among sector leaders and community-based
    organizations and use of community health workers to engage patients in their own care. The Dell
    Medical School Team will focus on integrating social services referral data with clinical data in a secure
    and scalable platform using well-tested privacy-protecting technologies and nationally-developed
    standards. The platform will be extended to a broader set of patients, conditions and social determinants of health at selected health clinics depending on each clinic’s availability to participate.
    The clinics will be selected through a process of community participation and collaboration throughout
    the project. All of the participating clinics will become part of the model community network.

The app will also protect patient privacy and security. Many apps have focused on health and electronic health records before, but they can be difficult to use and do not fit the needs of the people who can benefit from them the most. We plan to have patients involved in every part of the process to develop this app, so it is easy to use and meets the needs of diverse groups, including people who are less likely to use or trust technology.

    Our solution is to connect existing networks, increase researchers/other key stakeholders’ knowledge of PCOR, and utilize various forums to discuss ideas, share results, and build partnerships for conducting PCOR. We will also engage funders interested in PCOR and MSAWs' health. Work with MSAWs will occur in the Midwest. Researchers, providers and funders will be engaged across the U.S.

    Short-term objectives are to:
    1. Identify and expand network(s) through which researchers and local and national organizations share and obtain information about MSAW health and utilize those avenues to increase key stakeholders interested and prepared to conduct PCOR

    2. Expand MSAWs prepared and interested in participating in PCOR

    3. Engage funders in discussing PCOR as part of their funding portfolios/strategic plans including MSAWs

    A longer-term objective is to increase (a) the number of researchers conducting PCOR with MSAWs throughout the process; (b) providers/agencies utilizing the results; and (c) funders including PCOR and MSAWs in their strategic plans.

    Vision: The Center for Youth Mental Health envisions a mental health system that truly meets the developmental needs and goals of adolescents and young adults.

    Mission: The Center works alongside partners to strengthen relationships among local, state, and community-based initiatives to identify gaps and systemic barriers that prevent adolescents and young adults from getting the appropriate care at the right time and place.

    Goal: To ensure that coordinated, effective, and right-sized care is easily accessible in Central Texas to support adolescents in becoming healthy, productive and independent adults.
