We report a case of recurrent ventricular Tachycardia (VT) in a 49-year-old male patient previously known to have an inferior myocardial infarction and hypertension, presented with ischemic chest pain accompanied by dizziness, hypotension, and tachycardia. An electrocardiogram showed monomorphic VT. A prompt synchronized electrical cardioversion under sedation has reverted the rhythm to the sinus. 🌟 Revolutionizing Cardiac Imaging: A Breakthrough Case by Dr. Olfat Kamel Hasan 🌟 We are thrilled to present this month’s standout case from the Canadian Association of Nuclear Medicine, authored by the esteemed Dr. Olfat Kamel Hasan. This case not only showcases advanced diagnostic techniques but also introduces innovative insights into myocardial perfusion imaging. Case Overview: A 37-year-old male patient with a significant family history of premature coronary artery disease and obesity reported atypical chest pain. Despite normal ECG and echocardiogram results, further investigation through a cardiac PET examination was warranted. Key Findings: - Dipyridamole Stress Test: Administered at 0.6 mg/kg, with no ischemic ST segment depression or significant symptoms observed. - Hemodynamic Response: Notable splenic switch-off, indicating effective pharmacological stress. - Perfusion Images: No evidence of inducible ischemia or myocardial scar. - Myocardial Flow Reserve (MFR): Normal global MFR at 4.4, with no regional abnormalities. Clinical Significance: This case emphasizes the value of myocardial perfusion PET imaging in diagnosing coronary artery disease, particularly highlighting the splenic switch-off (SSO) phenomenon. SSO, characterized by reduced splenic tracer activity during stress, is a reliable marker of adequate pharmacological stress, improving diagnostic accuracy. Implications for Practice: Dr. Hasan’s findings highlight the importance of SSO in enhancing the interpretive accuracy of myocardial perfusion imaging. By identifying non-responders to stress agents, SSO helps prevent false-negative results, leading to better patient outcomes. Shaare Zedek’s Anat and Amnon Shashua Breast Health Center recently introduced a groundbreaking new approach to breast surgery that is achieved without general anesthesia, allowing for a faster and easier recovery. The collaboration between Dr. Yahelli Miller, head of the Breast Surgery Unit, and Dr. Ariel Grass, a senior anesthesiologist specializing in regional anesthesia and pain management, has led to the development of a unique surgical approach for the benefit of women requiring breast surgery. This development involves the use of ‘nerve blocks’ in breast surgeries combined with the administration of only sedative agents (sedation) to achieve a deeper-than-usual sleep, providing patients with a calm and pleasant surgical experience and significantly reducing, or even preventing, postoperative side effects and pain. Dr. Miller explains: "Today, breast cancer treatment is personalized for each patient according to her specific type and stage of disease. We have now also added the personalization of anesthesia. It’s important to stress that surgery without general anesthesia is suitable for many women, but not all. Through the collaboration between our departments, we have maximized the effectiveness and scope of regional anesthesia and achieved pain control in all types of surgeries, even lengthy ones that include breast reconstruction. Patients wake up from surgery quickly, feeling alert and overall good, and are able to return to a high degree of functioning very soon thereafter. Dr. Grass adds: "While traditional breast surgeries typically use opioid pain relievers like morphine, the approach we have developed at Shaare Zedek in using 'blocks' to achieve complete pain control without the need for long-term opioid administration during and after surgery, as well as reducing the amount of anesthetic agents and the necessity for artificial ventilation."
