Compared to demographically similar adults without ACEs, those with ACEs had substantially higher utilization and 26.3 percent higher expenditures. The aggregate spending difference across the 157.6 million US adults with ACEs was $292 billion in 2021. What are the main issues for Physicians (and I would venture to say Behavioral Health Providers as well) to treat Addiction you ask? This systematic review of 283 articles cited institutional environment (81.2% of articles) was the most common reason given for physicians not intervening in addiction, followed by lack of skill (73.9%), cognitive capacity (73.5%), and knowledge (71.9%). My thoughts here: 1. Institutional Environments that facilitate delivery of Evidence-Based Addiction Care: PAYMENT MODELS that allow the interdisciplinary team to function as a whole not as separate roles. Systemic support with real monetary value on the work and units. 2. Lack of skill and Knowledge: Training and education opportunities for physicians to practice necessary skills are essential- Project ECHO models or other educational development to close learning gaps across professions. 3. Cognitive Capacity: It is hard to have capacity when it is going to 1 million other things in patient care including battling burnout and fatigue. Back to systemic support. This. This! This is the kind of stuff that needs to be taught to ALL medical, nursing, and other healthcare students. Most of us get ZERO of this education on payment models or the financial nuances to healthcare and then end up working in this healthcare system. Sharing this link to KFF's Mental Health in America survey, which highlights how pervasive mental crisis events are, how complex they are, and how they are one of the worst things that can happen in a family. My heart breaks for these families. KFF CEO Drew Altman writes more in the essay. As you read, consider 25% of 342 million people is 85 million people... - 28% of Americans say they or their family had to take a painful step like institutionalizing a family member because they were a threat to themselves or others. - 21% say they or a family member had a drug overdose requiring an ER visit. - 14% say they or a family member ran away from home and lived on the streets due to mental health issues. - 16% say a family member experienced homelessness because of a mental health problem. - 8% say they or a family member had a severe eating disorder requiring hospitalization or in-person treatment.  - 26% say they or a family member engaged in cutting or self-harm behaviors - 16% had a family member who died from suicide. This a complex crisis for millions of individuals, and as Drew says, "multiplies manifold" as it affects families. Every chance you get, advocate your elected officials for more and better mental health services for your state.
