There could be many reasons as to why the project was not delivered on time, internal/external factors, issues beyond the team's control, etc. So first and foremost acknowledge the main problem and then try to focus on what went wrong, what could they have done differently and what measures they can take to mitigate risks so it does not happen again in the future.
