Calling all #biocoordinators #accesscoordinators #patientadvocates #reimbursementmanagers #arm #frm #fas #fras : What is everyone doing for their #PriorityHealth patients that were enrolled in #SaveOnSP who are now being discontinued as of June 1, 2024?!?! Those patients copay card funds have already been #EXHAUSTED as they had a #maximizer plan so will need to submit for #patientassistance and many may not qualify due to exceeding income requirements. What about offices that don’t have a dedicated point of contact? This is outrageous, should be illegal as they’ve taken all the copay card money up front (correct me if I am wrong) and is going to affect patients #access to therapy and putting patients at risk of #lapse in therapy and #discontinuation. My thought process as I was #brainstorming this issue is: -Contact reps to get #samples -Update VM asking pts affected by this to leave their name, DOB, name of drug and when they’re due for their next dose to triage who needs a sample first -Direct patients to manufacturer patient assistance foundations website to initiate the #PAP application ASAP -Create a checklist of what patients need to provide for their patient assistance application, especially if they want to do a paper application and have office submit -Say a prayer and hope for the best Any other suggestions?! #navigatingaccessissues #howisthislegal #michigan #thisisNOTok

EQIP – PC                                              A strange acronym               But a great model for health equity and independent primary care delivery Here is the important information- followed by the more mundane details below: -         An innovative Maryland Advanced Primary Care and Health Equity Funding program -         Infrastructure funding to build new advanced primary care sites in areas that are medically underserved, have high Area Deprivation Indices and high rates of avoidable hospitalizations. -         Intention is to attract and establish new primary care providers, as opposed to shifting around current providers -         Non-visit-based Medicare payments in addition to Medicare Fee for Service payment -         Blending into Medicaid VBP beginning in 2026 -         Strong inclination toward independent entities, as opposed to corporate owned or supported entities -         Focus on health equity, community health workers and behavioral health integration and linkages to community-based services -         Five-year model that progresses to integration into the larger Maryland Primary Care Program -         Free linkages to the state’s Health Information Exchange (CRISP) and AI event prediction advanced health informatics tools At a time when vanishingly small numbers of primary care residency graduates are choosing to establish their own practices, this thoughtful program’s features are ideal to attract young primary care physicians who have always dreamed of moving to an area where they are greatly needed and appreciated. Even better, the startup costs are funded by the state, and they will be given abundant technical support to build a strong modern practice.   Here is how the program fits into the state healthcare delivery system.  The State of Maryland, the Maryland Department of Health, the Health Services Cost review Commission, and the Center for Medicare and Medicaid Innovation released an application for the Episode Quality Improvement Program- Primary Care (EQIP-PC) program. This program was derived from waivers within the state’s overarching Total Cost of Care contract and will be managed by the Program Management Office of the Maryland Primary Care Program, under the capable leadership of the Executive Director Chad Perman. Perhaps the most important message from this collaborative work of the State Health Department, the state HIE (CRISP), Hospital Rate Commission, and CMMI is the intentional creation of a “health delivery system” with solid leadership and broad stakeholder input. 💊 When I was tasked by Mr. Mark Cuban and Alex Oshmyansky, MD, PhD a while ago to build the Cost Plus Drugs Affiliate Pharmacy Network, I agreed on the condition that we'd provide mechanisms to never drown #pharmacies on our ingredient costs. We all know specialty medications are expensive, but whats more expensive is poorly managed specialty medications. Programs like this from OptumRx have the potential to really help patients achieve the best outcomes while managing avoidable costs. For example, improving adherence to specialty meds for asthma/ copd can help reduce readmissions/ ER visits and prevent further disease progression. #valuebasedcare #specialtypharmacy #populationhealth

MY FELLOW ADVOCATES & LEADERS ⬇️ 😡 ⬇️ 😡 ⬇️ 😡 It has come to my attention that many of the life science event companies are exploiting my name by listing me as a confirmed speaker on their 2024/2025 event websites. This is clearly an intentionally deceptive tactic to leverage my reputation to solicit overinflated registration fees and “cash-grab” sponsorship integrations. What's more shameful is that I am not the only one this is happening to, and it has apparently been a deleterious corporate practice for years. The law requires that individuals must give their permission before their name or likeness can be used for a commercial purpose. By taking advantage of our collective names, these companies (and I am explicitly calling out Paradigm Global Events) may face serious consequences. Regardless of any prior appearances, giving off the impression that we are confirmed speakers for an upcoming event is a direct violation of our rights and the lowest form of unprofessional business practices. The event in question right now does not deserve the dignity of being mentioned, so if you are tagged below, your name may be currently exploited as I write this. DM me for details.
