    🎩 He’s worn so many hats since making his way to Wellvana in 2021. Here’s a new one for Readus Smith — President for National Partnerships U.S.  No one is better suited to lead Wellvana’s new venture, the Wellvana Interfaith Network, bringing the benefits of value-based care to faith communities, than Readus.  If you’ve ever been in a meeting with Readus, you know he leads with his heart. He’s a force of nature while being a friend to all.  🩺 An experienced healthcare executive and entrepreneur with a passion for bringing world-class care to underserved communities.  💯 A relentless optimist who won’t rest in pursuit of a worthy goal.  🫵 A charismatic leader who delivers on the hype. While value-based care is his vocation, his calling is as a lay leader in the Church of God in Christ. He serves as the Pentecostal denomination’s Secretary General of Health and Business. The appointment means he oversees the wellbeing of millions who are members of local congregations in all 50 states. And now his vocation and calling will come together.  He’s ideally suited to help the 5,000 doctors who are COGIC members - and thousands more clinicians in other faith communities - to transition from volume-based fee-for-service to patient-centered value-based care and bring new benefits to their communities.  It’s a novel partnership, but we hope it is the first of many as we spread the gospel about the Wellvana Interfaith Network.  (More details in comments).     When Nelsabeth Gonzalez completed medical school in the Dominican Republic, she worked for three years in primary care then moved to the United States where she fell in love with value-based care.   Her initial plan was to begin a residency in order to practice medicine in the U.S., so she started working as a medical assistant. Soon, she found herself tapping into her primary care background from the D.R.    Because she had the clinical fundamentals, Nelsabeth was able to help Accountable Care Organizations navigate risk adjustment and clinical documentation accuracy. This is how Medicare knows what kind of resources each patient will require each year. Without accurate risk adjustment, value-based care can’t work. During Hispanic Heritage Month, we honor Nelsabeth’s way to Wellvana and her contributions to transforming the U.S. healthcare system.

    📊 Few industries are creating as much data as healthcare.   Nearly a third of data volume is now attributable to all the technology surrounding patient care, according to one popular estimate. And IDC estimates roughly 270 GB of healthcare and life science data is created per patient in one year.     While making sense of this data explosion grows more challenging by the day, the opportunity is off the charts.   Imagine if every PCP: 🏠 Had access to a list of potential social determinants of health for patients.  🏥 Received an alert when a patient visited the ER in order to schedule a timely follow-up.   🔮 Knew which patients were at higher risk of disease and could use additional attention. Our analytics experts are committed to using all we’re learning about patients to improve their care and reduce costs in the process.     🤕 One wrong number is all it takes.   A patient in Michigan was hospitalized in July and ended up septic. Before going home, he spent some time in a skilled nursing facility.   Upon discharge, Crystal Lemos, RN, called to support the patient as a standard practice in Wellvana’s Foundational Care. Our nurses always ask about medication at times like these since changes in medication often lead to confusion.   This patient didn’t know what he was taking.   A few more questions revealed he also didn’t have any help at home. And he didn’t know if home health was coming to assist.   So Crystal traces back his steps through the health system. She discovers the SNF did order home health. But when she calls the home health agency, she learns that they couldn’t reach the patient, even after several attempts.   Just to be sure, she confirms that the agency has the same phone number for the patient that she has.   There’s the disconnect. Wrong number.   Without this kind of persistent support after hospitalizations, Medicare patients are more likely to wind up back in the hospital. Wellvana has seen a reduction in readmissions for practices who participate, on average removing approximately $1,200 in cost per patient from the healthcare system.    https://lnkd.in/eHyxBPvD

    🔮 There’s no crystal ball in value-based care. We have actuaries. And we’re welcoming two more to our growing team. Vice president Kevin Dotson brings more than two decades of actuarial leadership, having led value-based initiatives at both payors and providers. Director Adam Slatky, FSA, MAAA is an actuary who specializes in value-based after a career similarly spent with both payors and providers. Actuaries rarely travel in pairs, but Kevin’s and Adam’s lengthy experience has overlapped not once but twice. “Success in value-based care requires more than good intentions to support our patient’s needs. We pair our historical data insights with emerging patient health conditions, then layer on an array of care management interventions to create more valuable care for everyone involved,” says Jay Hazelrigs, Wellvana’s Chief Actuary.     Medicare patients have largely been left in the dark about the transformational shift underway to make their care more valuable. CMS has recognized this knowledge gap and responded with the creation of its “Value-Based Care Spotlight” page aimed at patients. Link: https://lnkd.in/eBxrsGh7 Value-based care requires behavior change from everyone involved — doctor, patient, and payor.
