    🤔 July is the month when primary care practices and health systems reevaluate their experience with value-based care. The deadline of August 1 to join an Accountable Care Organization puts one thing in perspective:    Accountable care is not something anyone can do alone.    This year, Wellvana partnered with the Healthcare Leadership Council (HLC) to author recommendations to advance (and simplify) policies related to accountable care.     “It’s been far too challenging for even the most sophisticated health systems to make the leap to value-based care,” says Kyle Wailes, Wellvana President and CEO. “Policymakers have an opportunity to bridge the divide so every patient can benefit from providers that are solely focused on keeping them well.”      We’re proud of our partners at AdventHealth who are thoughtfully investing in value-based care to transform a system of sick care into one focused on whole health and wellness.    “Until somebody cures all diseases, we’re going to still need hospitals,” AdventHealth President and CEO Terry Shaw says. “But the hospital should be the last whistlestop on the healthcare train, not the first.”    Read more from our joint report:    https://lnkd.in/eXc8UePx

    A patient in South Texas had run out of her anti-psychotic medication three weeks ago. She went to the ER because insurance denied her medication. She couldn’t get a refill.   The ER gave her one dose.   Wellvana’s Care Team received the discharge alert and sprung into action.     By this time, she said she was hearing voices again, and they were not telling her good things.   A Wellvana RN and a Licensed Clinical Social Worker figured out there was some confusion about this patient’s anti-depressant and her anti-psychotic — two very different medications.    Once the wires were uncrossed, the primary care practice pushed through the patient’s appeal. She was able to get back on her medication and thanked Wellvana. “Who knows what would have happened,” she said. The patient is now working closely with Christine Empeno, RN, who has been able to help this patient navigate other medication challenges as they pop up and keep her from an unnecessary hospitalization. Patients never pay for Wellvana’s discharge support. And our latest study found it’s reducing 30-day readmissions, increasing in-office follow ups and reducing per-patient medical costs by $1,100.     Practicing Primary Care Physician and Value-Based Care Leader

    When I was training to be a urologist, I was working for the department chair at Boston Children’s, the late W. Hardy Hendren, M.D. 👨⚕️ This was THE guy. Hendron did surgeries no one else would do. Some would last 12 or 15 hours. He earned the nickname “Hardly Human,” which even got a mention in his obituary by the American College of Surgeons. Link: https://lnkd.in/g6yANT6z One day, this world-renowned surgeon handed me his business card after a discussion about a patient. He told me to call that evening and let him know what I was thinking. I saw his home phone number (this was pre-cellphone) printed on the cards he gave to every patient. No way, I thought. “Is this really your home number?” I asked. “If a patient wants to find you, they will. And they’ll come looking before they really need you,” Dr. Hendron explained to me. “But if they know they can reach you if they need to, they'll wait until they really do.” 🤯 Wow. A thought I had never considered. It changed my entire approach. I freely give out my number to patients. No regrets. To other MDs, here's a multiple choice survey: Who gets your personal number? 📟 No one. That’s why I still only carry a pager. 👂 Patients who actually listen to me. 🚨 My staff, for emergencies only. 👨⚕️ Everyone. I’m going full Hendren/Rosenberg. Bring it on. Thirty years later, the only non-family photo on my desk is of Dr. Hendren. I can think of no better way to honor him than to encourage others to follow in his footsteps.

    On a night 25 years ago, our Chief Medical Officer, Matt Rosenberg, MD, had one of those “aha” moments that would inform his approach to primary care for decades to come. 💍 It was the night he got engaged. He was already in private practice delivering primary care in Jackson, Michigan. He made it very clear that he was not on call that night. And then he was called into the hospital anyway. A patient was dying, and his family was refusing intubation until they saw the doctor they trusted.     🛌 Our nurses have to learn a skill that’s similar but not the same as working at the bedside. Supporting patients when there’s no body language or eye contact can be frustrating. But they find ways to connect. And that’s when the benefits kick in. Communication reveals they can’t get to their next appointment. Or they can’t afford their medication. Or they’re concerned about a new symptom. Our nurses are on it, every day, helping patients through it all. Thanks to Olga Mosy-Madugba and all of the nurses leading the way to Wellvana. #nursesweek

    🧠 You cannot overstate the emotional intelligence of nurses. They know when to listen, when to speak, and even when to offer silence. As our nurses support patients between appointments, it’s not uncommon for their calls to last an hour or more. Patients have much more on their plates than what usually appears on their medical record. #nursesweek
