When you are so far from the edge you only hear crickets....this site has a 400' vertical drop with limited anchors near the edge. When this team has to haul a victim back up, they usually opt for remote anchors to facilitate a large haul field. After action review, debrief, post incident analysis....whatever you wanna call it. These should be mandatory after every incident response AND training evolution. At minimum, we ask every student; 1) What did we do well? 2) What could we do better? Some additional questions may be; 3) Did anything unsafe OR potentially unsafe happen? 4) If yes, what was done to mitigate the hazard? Encourage an environment where it's ok to say "I messed up".
