Last Friday, we hosted an in-person community event to bring together organizations from our #UniteVirginia network! 📍 Hosted at the Bon Secours Richmond Community Hospital Inc. in the Center for Healthy Living Sarah Garland Jones Center, 40 community members came together to network and discuss goals for the region. Focusing on their collaborative efforts, they explored how to leverage the Unite Us platform and the Unite Virginia network to create connections for the #patients and #Virginians they serve. During the meeting, we also spotlighted the great work at Senior Connections, The Capital Area Agency on Aging, an organization providing vital programs for #olderadults and is accepting referrals through Unite Virginia. Thank you to all the amazing organizations that came to learn, network, and show their support! 🤩 It was an incredible event, and we hope you gained valuable insights and strategies for enhancing access to care throughout the vibrant state of #Virginia. 💡 P.S.—#Didyouknow the Sarah Garland Jones Center is dedicated to Dr. Sarah Garland Jones, the first African American and first woman licensed to practice medicine in the Commonwealth of Virginia, and one of the founders of the first hospitals in Richmond allowing African American doctors to practice? She was a pioneer in the medical field, a champion for civil rights, and a role model for future generations. Her dedication and courage are an inspiration for us all to continue working together to transform #socialcare delivery across the country.

It was incredible to hear from leaders across the industry about their collaborative work addressing Veterans’ mental and behavioral health and working on Veteran #suicideprevention initiatives at the U.S. Department of Veterans Affairs/ United States Department of Defense Suicide Prevention Conference last week. VA Secretary Denis McDonough spoke about the VA’s 200-point plan and federal action plan, setting the stage for a great discussion for the rest of the conference. Plus, Special Assistant to the President for Veterans Affairs, Terri Tanielian’s keynote address was inspiring! She spoke about the White House’s priority goals and highlighted the 2023-2024 administration commitments, including: ✅ Enhancing community-based prevention by supporting the Governor's Challenge ✅ Improving lethal means safety ✅ Expanding outreach to justice-involved Veterans ✅ Expanding access to legal support services ✅ Providing financial support ✅ Expanding access to peer support services In alignment with these commitments, Dr. Halima Ahmadi-Montecalvo, PhD, MPH presented on the importance of public-private partnerships to help prevent Veteran suicide, with a focus on the implementation of tech-enabled social care coordination. Unite Us is proud to partner with organizations across the nation that work daily to address the health-related social needs of Veterans, Service Members, and their families. 💙

It's time ⏰ to unpack the difference between Social Drivers of Health (#SDOH) and Health Related Social Needs (#HRSN). While related, and often used interchangeably, there is an important distinction to be made between the two...📝 💡 #SDOH are the conditions in which people are born, grow, live, work, and age that impact health outcomes, such as socioeconomic status, education access, neighborhood/physical environment, and more. 💡 #HRSN are the immediate needs that affect people's ability to maintain their health and well-being, such as housing stability, food insecurity, access to reliable transportation, and more. Here's an example to help illustrate the difference: 🔎 SDOH = Individual lives in a neighborhood classified as a food desert. ✅ The intervention = Building a grocery store in that neighborhood. 🔎 HRSN = Individual has low food security. ✅ The intervention = Enrolling that person in a food delivery program At Unite Us, we partner with organizations that address both upstream and downstream needs to reduce health disparities and foster healthier communities.
