We were are one out of about 400 participating organizations building Dementia Care Programs (DCP) to serve our communities in this innovative program. Contact us to learn more!

    CMS is pleased to announce the launch of the Guiding an Improved Dementia Experience (GUIDE) Model – the first model of its kind to reach and provide support for people with #dementia and their #caregivers. Through the Guiding an Improved Dementia Experience (GUIDE) Model, nearly 400 participating organizations building Dementia Care Programs are serving hundreds of thousands of people nationwide: https://lnkd.in/eUNJ-B-w.     The aim of care coordination is to promote communication and continuity of care across providers, specialties, and medical systems, ultimately reducing healthcare costs and improving clinical outcomes. This is crucial for managing both complex acute care and chronic illnesses. Older adults, often facing multiple chronic conditions and navigating various specialists, experience significant challenges. About 42% of older adults perceive poor care coordination, with ~15% reporting conflicting advice from different providers. Only one-third have met with a care coordinator.
