Ask The Expert - Question: When conducting a risk assessment, what data do I use when considering history of injuries or accidents on a machine when we have non documented? Answer, from Keith Brettin 🔽 When needing a #machine history look for similar machines or process in your facility. Operators are also a valuable source of information. Unfortunately, near misses may have not been documented as some do not report injuries because they may consider them minor, part of the job, or afraid of reprisal for having a recordable incident. Next, see if other facilities the company owns has any history on similar machines. Finally, you can look to OSHA at https://lnkd.in/er2AeRxF where you can utilize several options for the best results. This tool can also be used to search for incidents or citations at facilities that have recently been acquired, to avoid repeat violations on future investigations either planned or unplanned.

Ask The Expert with Schmersal’s Devin Murray, BSEE, MBA, FS Eng (TÜV Rheinland) - Question: What is MTTFD or Mean Time to Dangerous Failure. Answer, from Devin Murray, BSEE, MBA, FS Eng (TÜV Rheinland) 🔽 In respect to ISO13849, the mean time to dangerous failure is the estimated operating time to a dangerous failure of a #safety component or #SafetySystem. Though #SafetyComponents are designed to fail-to-safe, the MTTFD considers the time in years, to when a safety component or system can fail in a dangerous state. This time is based on the type of component used and the operating demand of the device/system. The MTTFD is one element used when determining the probability of dangerous failure per hour (PFHD) which equates to the Performance Level within functional safety.
