During the visit, we discussed the current healthcare needs of our community and explored ways to improve access to essential services. About us
PrimeCareHealth (PrimeCare) is a federally qualified community health center (FQHC) that provides quality, affordable care to medically underserved populations on Chicago’s West and Northwest sides. Over its 22-year history, PrimeCareHealth has built a community-inspired, nationally recognized health care system that provides comprehensive family medical care at its five accessible locations, including health services for adults, seniors, women and children, as well as prenatal health services, behavioral health counseling, and case management, regardless of patients’ ability to pay. Our clinics and health centers are staffed with trained medical professionals, including physicians, residents, nurses, case managers, care coordinators and service support staff. Our Behavioral Health services are fully integrated into our health care. Our licensed clinical psychologist and several licensed counselors work closely with our medical providers to support patients dealing with chronic conditions and diagnosed mental health issues. Included in our approach: • Family Medicine-model of care for patients of all ages, from newborns to seniors • Strong Emphasis on prenatal care and complex chronic disease care, including case management and care coordination • Seamless provider transition between care settings – primary care, specialty care, acute inpatient, rehabilitation and long-term settings. • Behavioral health services for children and adults integrated into primary care • Outreach and Enrollment services for Medicaid expansion and Marketplace clients • Culturally and linguistically competent care, with emphasis on underserved Latino and Polish communities – more than 65% of patients are served in a language other than English.

On June 27, Guadalupe Salgado, our Senior Manager of Care Coordination, and Julie DeJesus, the Clinical Nurse Lead at Northwest, had the honor of attending the Innovation Grants Event: Instigators of Change, where we were proudly recognized as Grantees! It was a fantastic gathering of representatives from federally qualified health centers, nonprofits, community-based organizations, faith groups, and our amazing partners. The event also marked the launch of the Beyond Barriers program. As part of this initiative, Beyond Type 1 will provide $100K in grants to local organizations in Miami, Detroit, and Chicago. These grants aim to support organizations driving meaningful solutions for the diabetes community and prioritizing care for underserved populations. This past weekend, PrimeCare Health joined Erie Family Health Centers Tapestry 360 Health Esperanza Health Centers PCC Community Wellness Center and ACCESS Community Health Network Inc and hosting a Hydration Station during this year's Chicago Pride Parade. Through this collaboration, we were able to provide water, treats, safe sex materials, Narcan, and information about all of our services to over 400 parade-goers. Thank you to all the participating organizations for contributing their time and resources to this event. We had so much fun celebrating Pride and supporting our LGBTQ community.

Last month, Sajia Aziz, MD, MPH, Quality Improvement Specialist at PrimeCare, Harini Gajjela, MD, and Juliet Bradley, MD, FAAFP, attended the STFM (Official) Society of Teachers of Family Medicine Conference in Los Angeles. The highlight of their experience was presenting their research, "Shouldering Hyperglycemia: Anticipating LGA Neonates with Abnormal Glucose Tolerance Tests" using data from PrimeCare (2019-2023). This research delved into the critical intersection of maternal health, public health, and obstetric complications. It revealed a positive correlation between glucose tolerance tests and the increased risk of shoulder dystocia during childbirth. A HRSA OSV (Health Resources and Services Administration On-site Visit) is an evaluation of a health center's compliance with federal standards.
