Want to hear the origin story of ATLS? Radiolab Podcasts WNYC Studios has you covered. Episode intro: February 1976. A flight out of California turned catastrophic when it crashed into a farm in rural Nebraska. What happened that night at the local hospital, and crucially, what went wrong, would inspire a global sea-change in how emergency departments operate and fundamentally alter the way doctors think in a crisis. BTW, the always-insightful Reuben Strayer, MD - emergency physician educator at Maimonides Medical Center - makes a cameo appearance. Listen to the full episode here: https://lnkd.in/ejzZy88d

The Commonwealth Fund recently released their 2023 Health Care Affordability Survey which looked into “the extent to which working-age adults say their insurance provider charged for a health service they thought should have been free or covered or denied coverage for care recommended by their doctors.” The top findings: - “Forty-five percent of insured, working-age adults reported receiving a medical bill or being charged a copayment in the past year for a service they thought should have been free or covered by their insurance.” - “Less than half of those reporting billing errors said they challenged them. Lack of awareness about their right to challenge a bill was the most common reason, particularly among younger people and those with low income.” - “Nearly two of five respondents who challenged their bill said that it was ultimately reduced or eliminated by their insurer.” - “Seventeen percent of respondents said that their insurer denied coverage for care that was recommended by their doctor; more than half said that neither they nor their doctor challenged the denial.” - “Nearly six of 10 adults who experienced a coverage denial said their care was delayed as a result.” Find the full study here: https://lnkd.in/gKZZsUmE

2023 was a good year for physician assistants, at least where salary is concerned. According to the Medscape Physician Assistant Compensation Report 2024: Total compensation, on average jumped by around 6% for US PAs last year. The biggest single component of that compensation is base pay, which rose by about 7%. Those hikes came on the heels of 4% and  3% bumps, respectively, in the prior year The way a PA is paid has a big affect on how much they are able to earn. “I think it’s super-important for PAs to understand what their billables and allocated overhead are when they go into contract negotiation,” Stephanie Doty, national director with Los Angeles-based HealthCare Recruiters International, says. “There’s a lot of room for negotiation, and they can be their own businessperson.” For more of their findings check out the full study here: https://lnkd.in/gmN3dpCd

Team trip in Utah this week to reflect on the progress we’ve made so far and plan for some big milestones we’re targeting to hit next. Building this job marketplace for emergency medicine clinicians is a lot like climbing these mountains. Lots of ups & downs & more ups - and the reward is worth it. Thank you for all of your support!

During the pandemic, many of us found solace in the unconditional love of a new pet. Yet, as we opened our hearts, an unseen force was tightening its grip on our wallets. Private equity firms now own nearly 30% of veterinary practices in the US. This consolidation isn't just changing ownership - it's fundamentally altering the landscape of pet care, often at the expense of our beloved companions and the dedicated professionals who care for them. Veterinarians report being overworked and pressured to upsell. Pet owners face skyrocketing costs, with prices for vet services rising 60% since 2014. It's a situation that's forcing many to make impossible choices between their pet's health and financial stability. Senators Elizabeth Warren and Richard Blumenthal are now demanding answers. They're investigating how this trend affects the quality of care, pricing, and working conditions in the veterinary field. It’s hard not to draw comparisons between this and what we’re seeing in emergency medicine. What's your experience? Have you noticed changes in your vet visits? Did you know that only 8% of emergency physicians now practice in rural areas, down from 10% in 2008.?This alarming decline underscores a growing crisis in access to emergency care for millions of Americans living in rural communities. From a recent Emergency Medicine News article: - 92% of clinically active emergency physicians practice in urban areas - Median age of rural EM physicians is 58-62, compared to 50 in urban areas - 16 states meet less than 50% of EM physician demand in rural areas Barriers to rural emergency care include staff shortages, transportation difficulties, lack of specialists and resources, and limited residency training in rural environments “"When you talk with doctors with resources, they have no idea what it's like to be without them. If I have a patient with a GI bleed and liver failure, traditionally the massive transfusion protocol would include red cells, FFP, platelets, and cryoprecipitate, but here the only blood products we have are red cells and platelets, so we do not have a full spectrum MTP,” Dr. Robyn Hitchcock We believe it’s time to address this growing disparity to ensure all Americans have access to quality emergency care, regardless of their location. What solutions have you seen work in your community? How can we collectively tackle this critical issue? Read the full article here: https://lnkd.in/eGCHRkqk

More physicians ascending to top executive roles within health systems and healthcare companies. Here's what emergency medicine clinicians should know: - Physicians are increasingly sought after for CEO and president roles in hospitals and health systems. - The COVID-19 pandemic has accelerated the need for care transformation, driving demand for clinical expertise in leadership. - Physician executives bring unique insights into value-based care and workforce issues. Benefits of physician leadership: - Bridge clinical and administrative perspectives - Better understanding of on-the-ground realities vs. paper metrics - Ability to "translate" between clinical and C-suite languages "I don't see how you can make this about value-based care without the care provider, because what value-based care really is trying to optimize is the patient outcome." - Steven Kalkanis MD, CEO, Henry Ford Hospital Challenges for physician executives: - Balancing clinical needs with business demands - Navigating organizational politics - Adapting to increased visibility "For physicians, I think we do have a special experience when you go through training and especially residency, and you take calls in the middle of the night, and you see people at their most vulnerable," Kalkanis said. "That somehow gets ingrained into our own professional DNA and helps inform leadership decisions when it comes to putting yourself in that moment." Do you have executive-level physician leadership where you work? What have the results shown so far? The 2024 Medicare Physician Fee Schedule (MPFS) changes are reshaping reimbursements, with a focus on rebalancing compensation toward cognitive services. Many physicians are deeply concerned that the government’s payment structure has led to worsening physician underpayment across specialties. CMS payments to physicians are not adjusted for inflation, steadily eroding the value of reimbursements. Meanwhile, the costs to run a practice are increasing. Add in sizeable med school debt that carries high interest rates, and physicians feel truly squeezed. Brief overview of the MPFS 2024 changes: - CMS has increased RVUs for cognitive evaluation and management (E&M) services to address historical undervaluation, aiming to better compensate cognitive care.  - A 3.4% reduction in the conversion factor is planned to offset higher E&M payments, impacting specialties differently. - Emergency medicine faces a projected 1% decrease in Medicare reimbursements, contrasting with potential gains for primary care. CMS has announced further cuts for 2025. How much more government cutting can physicians take? Read the full article here: https://lnkd.in/dfeehaMd
