Helping oil and gas leaders reduce major process safety incidents and improve operational performance using proven lessons from the US Nuclear Navy. Founder & President at High Reliability Group LLC | Keynote Speaker

If you work in Refineries or Chemical Process Plants, take some time to read the final report from the U.S. Chemical Safety and Hazard Investigation Board regarding the September 2022 fatal fire at the BP-Husky (now Cenovus' Ohio Refining Company) Refinery. HRG teaches "Learning Organization and Level of Knowledge" as principle number 1 of our HRO model of culture. Unfortunately, companies continue to underinvest in becoming a learning organization. See the excerpt below from the executive summary. After you read this report, please let me know why your organization does not need to focus on High Reliability Organization principles. {Yes, I am tired of being nice about this...people are getting killed.} We can do better. We must do better. Let's learn and apply HRO Principles. "Learning from Incidents. The final recommendation of the report of the BP U.S. Refineries Independent Safety Review Panel in 2007 (“the Baker Panel Report”) stated: “BP should use the lessons learned from the Texas City tragedy and from the Panel’s report to transform the company into a 13 Investigation Report recognized industry leader in process safety management” [2, p. 257]. a In its investigation of the September 20, 2022, naphtha release and fire, the CSB found similarities between the overflow events of the BP Toledo Refinery incident and the findings from the fatal explosions in 2005 at the BP refinery in Texas City, Texas. Catastrophic incident warning signs existed prior to the September 20, 2022, incident at the BP Toledo Refinery, and had BP effectively recognized and acted upon the warning signs following a 2019 incident, the company could have provided more effective safeguards to prevent the overflow of multiple vessels during a refinery upset such as the September 20, 2022, incident. (See Section 4.4)" P.S. Did you take action to share the learnings from this incident with your team? Or did you just say to yourself "That can't happen here. We are too good."? https://lnkd.in/gCiWJt84 #highreliabilityorganization #processafety

  • In light of recent high-profile incidents investigated by the Chemical Safety Board (CSB), the importance of designing effective safety and training programs is now more crucial than ever. In the midst of National Safety Month, it's the perfect time to reassess and reinforce our unwavering commitment to safety and operational excellence. Some items to think about during Safety Month: 1. External Reviews When incidents occur, however minor, are you unleashing the right external review? Picture an unbiased team conducting a thorough 72-hour on-site evaluation, meticulously observing shift turnovers, teamwork, and communication practices. The insights gained from these assessments are game-changing, revealing operational gaps and providing actionable recommendations that drive real improvement. 2. Leadership Unity Are your team of leaders unified in their vision for safety, all having participated in HRO workshops? These transformative sessions help develop a cohesive vision, ensuring safety isn't just a policy but a core value embedded at every level of the organization. 3. Workforce Training Workshops can train your frontline leaders on HRO principles using real-world case studies and foster an environment of continuous improvement. For example, when your organization adopts the "questioning attitude" and "team backup" principles, employees become proactive problem solvers, contributing to a safer and more reliable operation. 4. Coaching Programs Consider individual or group coaching programs that focus on specific problem-solving, where leaders are mentored to build confidence and adhere to HRO principles. 5. Audits and Reassessments Regular third-party audits and reassessments, coupled with root cause analysis for every incident, help maintain a high standard of operational excellence. This proactive approach ensures small errors are identified and corrected before they escalate, fostering a culture of vigilance and proactive management. Investing in these areas isn't just about compliance; it's about building a culture that prioritizes safety and excellence at every level. The key is to select the right partner in this journey, ensuring that your organization not only meets but exceeds safety standards. #OperationalExcellence #NationalSafetyMonth #ProcessSafety For more information on how High Reliability Group (HRG) can help your organization, contact us here or visit our website.

  • Creating a Zero Incident Culture: Insights from the High Reliability Group In the pursuit of Process Safety and Operational Excellence, industries often turn inward, referencing "Industry Best Practices" while overlooking valuable lessons from other high-risk sectors. A standout example is the U.S. Navy Nuclear Propulsion Program (NNPP), which has maintained a ZERO incident record over its 65 year history. This exemplary safety performance is not accidental; it is the result of rigorous leadership, continuous learning, and a deep commitment to operational integrity. At High Reliability Group (HRG), we draw on this unparalleled record to help organizations achieve extreme operational excellence. Here are five key organizational behaviors that set the NNPP and other High Reliability Organizations (HROs) apart: 1. Develop Leaders Today for Tomorrow: Invest in leadership development to ensure future success. 2. Do Everything with Honesty and Integrity: Maintain high ethical standards in all operations. 3. Learn and Teach: Foster a culture of continuous learning and knowledge sharing. 4. Strive for Excellence: Always aim for the highest standards in performance. 5. Hunt for Ways to Get Better: Constantly seek improvements and innovations. Rickover's leadership principles, which include rigorous training and unwavering commitment to safety, are integral to creating a Zero Incident Culture. The Nuclear Work Model, which emphasizes procedures, training, and supervision, ensures that operations are conducted safely and efficiently. This model can be adapted to any organization dealing with complex and high-risk environments. Creating and sustaining a Zero Incident Culture is not just about setting lofty goals; it's about providing the support, training, and leadership necessary to achieve those goals. At HRG, we help organizations implement these principles to transform their culture and achieve remarkable safety and operational performance. If you're ready to elevate your organization's safety and operational standards to the level of the U.S. Navy Nuclear Propulsion Program, connect with us at HRG. Let's work together to build a safer, more reliable future. #HighReliability #ZeroIncidentCulture #OperationalExcellence #Leadership #ContinuousImprovement For more insights on operational excellence and creating a high reliability organization, reach out to our team. • Honoring Memorial Day: A Commitment to Safety and Operational Excellence As we approach Memorial Day, a day dedicated to remembering and honoring the sacrifices of those who have given their lives in service to our country, it's also the perfect time to reflect on how we can honor their legacy through our actions. This Memorial Day, let's take the opportunity to reinforce our commitment to safety and operational excellence within our own teams. Memorial Day, observed on the last Monday of May, marks the unofficial start of summer. This year, it falls on May 27, signaling a season of heightened activity and increased operational risks in our industry. As we ramp up projects and maintenance schedules, ensuring the safety of our operations becomes even more critical. Here’s where we can make a real difference. Sharing the principles of High Reliability Organizations (HROs) can help spread a culture of safety and operational excellence. Think about the powerful impact we can have if we each commit to these principles and encourage others to do the same. Here are the key pillars that we need to focus on: 1. Leadership - Safety starts at the top. Leaders must be engaged, accountable, and focused on the most critical issues affecting process safety. By demonstrating a commitment to safety, leaders can set the tone for the entire organization. 2. Culture - Building a strong safety culture requires ongoing effort and dedication. Leaders must model the desired behaviors, promote a questioning attitude, and ensure integrity in all actions. This means not just following procedures but understanding and improving them. 3 - Systems and Structure - Effective management systems and organizational structures support the culture of safety. These systems should reduce variability and provide a consistent approach to managing risks. 4. Feedback - Continuous improvement relies on honest feedback. Organizations must learn from both successes and failures, using this feedback to refine processes and enhance safety measures. By embracing these principles, we can create a ripple effect of safety and reliability that extends beyond our own teams. As you reflect on the meaning of Memorial Day, take a moment to think about how you can contribute to a safer working environment. Share these principles with your colleagues, discuss them in your meetings, and make safety a topic of everyday conversation. Let’s honor the memory of those who have served by committing to the safety and well-being of those who continue to work in challenging environments. This Memorial Day, let's not only remember the past but also shape a safer future together. Spread the word and let's make operational excellence a shared commitment. As we move into the second half of 2024, let's keep this focus on safety and #operationalexcellence at the forefront of our minds. Together, we can ensure a safer, more reliable working environment for everyone. #HighReliabilityOrganization #safety
