State Medicaid agencies recognize the importance of addressing health-related social needs to improve health outcomes, but need help to advance social needs screening policies and programs. Working with Center for Health Care Strategies and Social Interventions Research and Evaluation Network (SIREN), we've developed a tool that outlines decision points for developing or improving screening policies. This tool is made possible by Kaiser Permanente through its National Community Benefit Fund at the East Bay Community Foundation. We had an impactful morning at the 2024 San Joaquin County Binational Health Event alongside Navneet Sandhu, MPH and Melissa Meza, MPH. Our team showcased the PATH Collaboratives HealthBegins leads in the Northeast, and San Joaquin/Stanislaus that provide CalAIM Community Support and Enhanced Care Management (ECM) services. We also highlighted the valuable services available to the community offered by our Collaborative members. This event strengthened our connections with local healthcare partners and helped us reach more organizations dedicated to improving community health outcomes.
