✍🏻 Connections Blog: Social Care Networks Announced Under New York’s HRSN Waiver ✍🏻 Yesterday we shared the news that Findhelp has been selected to partner with several Social Care Networks (SCNs) as part of New York's 1115 Medicaid waiver. Today, we're sharing more information and highlighting one of our SCN partners, SOMOS Community Care 🗽 SOMOS is a network of over 2,600 culturally competent healthcare professionals who provide care to over 1 million patients in New York’s underserved neighborhoods—particularly Asian, African-American and Latino communities. Their network comprises primary care physicians, specialists, behavioral health providers, pharmacies, community-based organizations. She has had the advantage of being a part of Piedmont’s Population Health model since inception in July 2010. ▶️ In May of 2015, Jennifer completed her black belt in Six Sigma Quality and Process Improvement certification. She is the Director of the Population Health department which consists of a multi-disciplinary team to include RN Care Managers, Resource Coordinators, LMSWs and PharmDs in which the team manages rising and high-risk patient populations across several commercial and Medicare Advantage value-based contracts covering over 400K patient lives. ▶️ Most recently, Jennifer has served as the business owner and instrumental in the implementation of a new social network platform across the Piedmont Healthcare system called Empowering You in partnership with Findhelp, helping their clinicians and patients get connected to community resources to address social determinant of health barriers in an efficient and coordinated way. 💬 We asked Jennifer what the future of social care looks like, and she said: "Social care would be a standard scope of practice across the care continuum and adopted within healthcare system quality and safety programs. Understanding the socioeconomic, physical, and environmental factors that our patients and communities face is critical as these factors play such a significant role in one’s ability to access the health system and self-manage chronic conditions for optimal health outcomes. Our Founder and CEO, Erine Gray, recently wrote an editorial for RealClearPolicy (part of RealClear Media Group). Erine highlights the potential dangers of selling social care data in the absence of a single, comprehensive federal law regulating what can and can't be sold, and to whom. Erine is a graduate of The LBJ School of Public Affairs at The University of Texas at Austin and is an expert on public policy and how it impacts social care. He calls for better regulation to prevent organizations from selling or licensing individually-identifiable social care information.
