Enhanced Care Management (ECM) is a statewide Medi-Cal managed care plan (MCP) benefit that provides person-centered, community-based care management to the highest need members. ECM Care Managers work as part of the member's care team and are responsible for: -Coordinating all aspects of ECM; -Coordinating with other care and case managers to ensure a seamless experience for the member and non-duplication of services; -Developing individualized care plans based on members' needs; -Motivating the member in achieving care plan goals and educating on their health conditions; -Guiding and educating the member and/or family and caregivers on the available resources, independent management of their health care and social needs, and self advocacy; -ECM Care Managers may also connect members with community-based services that help address health-related social needs, such as: Housing support, Medically tailored meals, and Sobering centers. As a Federally Qualified Health Centers, Elica provides comprehensive behavioral health services. At Elica Health Centers, patients have access to the following interternal Behavioral Health services by simply requesting a referral from their Elica Primary Care Provider 💜 : - Psychotherapy and social work services by Licensed Clinical Social Workers and Associate Social Workers; - Psychiatry services for evaluation and medication management in office and via telehealth; - Case Management services to address social determinants of health (a part of primary care services); - Substance Use Disorder treatment including counseling and Medication-Assisted Treatment (Suboxone, Naltrexone, etc) for opioid use disorder, alcohol use disorder, and others.
