Our mission is to provide high quality, affordable health care through the operation of professional and compassionate health homes for underserved populations in the region, who are confronted with barriers to accessing care.

A recent study revealed that more than 1 in 10 FQHC patients are impacted by social risk factors. While funding that supports FQHC's efforts in meeting patients' social needs is only starting to pick up and usually comes with disproportionate administrative burden, some of those safety net clinics have been addressing the Social Determinants of Health (SDOH) of their patients and community for many years. At Elica Health Centers, we operate a Resource Center that houses a Community Closet and a Food Pantry. All community members are welcomed to stop by for clothing, household items, fresh and pantry foods, information on resources in the community, as well as personalized assistance in becoming our patients and navigating the health care system. When patients are seen in clinic or over the phone, when discussing their referrals with referral coordinators or speaking to any other Elica staff, they are screened for SDOH or may present with a barrier during those interactions. Providers and all other staff may recommend Care Coordination and send a referral to our Social Care team. Our Social Care team of trained Care Coordinators, Enhanced Care Management staff, and Coordinated Access Navigators are ready to navigate the patients through resources and the health care system, direct patients to appropriate housing resources, as well as plan and support patients on their journey to physical, mental and social wellbeing. Elica Mobile and Street Medicine units are ready to "meet patients where they are." The Health on Wheels mobile medicine program features state-of-the-art mobile health clinics with full exam rooms that provide services to two primary groups: hard to reach populations with barriers to accessing care and underserved students at select schools within Elica's service area. https://lnkd.in/eirx3Frz

Yes, mental health issues can affect anyone, regardless of age, gender, race, ethnicity, religion, income, social status, sexual orientation, or other aspects of cultural identity. So why is Minority Mental Health being highlighted this month? Minorities face unique challenges when it comes to Mental Health. Obstacles for many include a lack of health insurance, less access to treatment, limited access to culturally and linguistically appropriate services, and stigma. Enhanced Care Management (ECM) is a statewide Medi-Cal managed care plan (MCP) benefit that provides person-centered, community-based care management to the highest need members. ECM Care Managers work as part of the member's care team and are responsible for: -Coordinating all aspects of ECM; -Coordinating with other care and case managers to ensure a seamless experience for the member and non-duplication of services; -Developing individualized care plans based on members' needs; -Motivating the member in achieving care plan goals and educating on their health conditions; -Guiding and educating the member and/or family and caregivers on the available resources, independent management of their health care and social needs, and self advocacy; -ECM Care Managers may also connect members with community-based services that help address health-related social needs, such as: Housing support, Medically tailored meals, and Sobering centers.
