About us

Community Care Partners (CCP) is a collaboration of two community-based providers, Bay Cove Human Services and Vinfen Corporation, that offers integrated care coordination to MassHealth enrollees who have complex medical and behavioral health needs. Our multidisciplinary Care Teams partner with enrollees to overcome barriers and meet their health and wellness goals while working collaboratively with healthcare providers and community based social services providers to better meet enrollee needs. Bay Cove Human Services and Vinfen Corporation are leaders in their local communities and fields of service.

  • CCP is proud to be a consortium of 4 leading community-based organizations: Vinfen, Bay Cove Human Services, Justice Resource Institute, and Mystic Valley Elder Services. Our member organizations offer rich histories of serving those most in need in communities across Massachusetts, including specialty populations such as those with mental illness and/or substance use disorders, development disabilities, pediatrics, the elderly, and the homeless. Our Bay Cove Human Services partners recently posted a video tour of their CASPAR shelters, which are part of an array of comprehensive services available to those affected by substance use disorders. At CCP, we proudly partner with MassHealth adults and children who have mental health, medical, intellectual, and developmental needs in order to break through barriers to living healthy, meaningful lives. People of all abilities should have the opportunity to act on their potential and thrive in their communities!

  • Don't forget, doula services are now covered by MassHealth! Doulas provide non-medical support to members and families during pregnancy, delivery, and up to 12 months after delivery. Doulas can answer questions about pregnancy and birth, connect to community resources, help prepare for birth, and provide a variety of supports during labor, delivery, and beyond.   • Summer Eats is a federal nutrition program designed to ensure that all youth (18 and under) have access to healthy meals and snacks during the summer when school is not in session! There are Summer Eats sites across Massachusetts that provide free meals to all kids and teens, no identification or registration required.   • CCP celebrates Community Health Improvement Week which runs from June 10 – 14 this year! #CHIWeek is a time to honor those working in partnership to improve the health and well-being of individuals and communities and advance health equity. CCP's care teams are deep in this type of work every day and every week!! By choosing to serve in the Community Partner program, our Care Coordinators, Nurses, and Clinicians support MassHealth Members with the highest needs. We appreciate each and every one of you!

  • This past weekend, Maddie Keating (Bay Cove CP Care Coordinator) and Erin Gadbois (Senior Program Mgr, CCP central team) tabled at BAMSI's first annual #Pride event in Brockton, MA! It was a pleasure to meet so many community members and other organizations providing valuable supports to those in need. We also learned about BAMSI's Transgender Community Outreach Resources & Empowerment (TCORE) Program which provides trans, non-binary, gender non-conforming, and other members of the #LGBTQIA community with access to safe and affirming services as well as resources and opportunities to live a life on their terms.   • This week, CCP staff attended a training on Cultural Humility provided by Jason Trefts of Boston Center for Independent Living. The practices of both cultural competency and humility were explored and contrasted. Strategies for implementing cultural humility within care coordination work were discussed as well. Staff came away with actionable knowledge to apply to our wonderfully diverse member population. We thank Jason and our partners at BCIL for their commitment to developing a skilled, compassionate, and culturally sensitive workforce!

  • #Healthliteracy is empowering and effective in giving people a voice in their own healthcare. Lack of health literacy is a barrier for our members as it can prevent them from understanding, receiving, and fully engaging in care. Improving health literacy promotes health equity and tools like the Improving Health Literacy for More Equitable Health Outcomes infographic from Center for Health Care Strategies provides organizations with a great baseline understanding of health literacy and strategies to enhance it.
