Under CalAIM, managed care plans may offer 14 Community Supports — services not traditionally covered by Medi-Cal that address social needs. Some of these services, like housing navigation, have been readily adopted, while others have gotten off to a slower start. We recently published a collection of fact sheets that outline implementation tips from organizations offering these less commonly adopted CalAIM Community Supports: 🔹 Asthma remediation 🔹 Day habilitation 🔹 Nursing home transitions/diversion 🔹 Personal care services and respite services 🔹 Sobering centers Check out the collection: https://lnkd.in/g9xkTp_7

Improving California's health care system, so it works for all Californians.
About us
The California Health Care Foundation (CHCF) is dedicated to advancing meaningful, measurable improvements in the way the health care delivery system provides care to the people of California, particularly those with low incomes and those whose needs are not well served by the status quo. We work to ensure that people have access to the care they need, when they need it, at a price they can afford. CHCF informs policymakers and industry leaders, invests in ideas and innovations, and connects with changemakers to create a more responsive, patient-centered health care system. A recent CHCF survey reveals stark disparities in health care access for Central Valley residents. The recent symposium in Fresno brought together community leaders, health advocates, and experts to address the findings: • 56% believe their communities lack sufficient mental health providers • 48% carry medical debt (vs. 36% statewide) • 35% report increasing difficulty accessing affordable, quality care • 50% face unreasonable wait times for physical health appointments • 78% have a regular doctor (vs. 85% statewide) These findings underscore the urgent need for targeted solutions to improve health care access and affordability in the Central Valley. I'm so proud to be working with California Health Care Foundation grantee California Black Health Network (CBHN), which is launching the statewide Black health equity campaign,"How Do I?" This effort builds on a large body of research conducted by EVITARUS in which Black Californians asked for information and tools to help them advocate for the high-quality, fair health care. The "How Do I" campaign, supported by materials designed by grantees Voice Media Ventures and content by Health Literacy Media, responds to that request. Please let CBHN CEO Rhonda Smith tell you a bit about it. https://lnkd.in/gC-YhNJb

🏥 Hospital Presumptive Eligibility (HPE) is a vital tool for expanding Medi-Cal access in California, but its potential remains untapped due to low awareness and confusion about its scope. Our recent study reveals significant barriers preventing HPE enrollees from transitioning to ongoing Medi-Cal coverage, but also offers some potential solutions: 🔹 Multilingual awareness campaigns 🔹 Streamlined application processes 🔹 Expanded assistance options 🔹 Culturally sensitive support We can bridge the gap between temporary and long-term coverage, ensuring that all eligible Californians have access to the healthcare they need and deserve.
