About us

The AC Care Alliance (ACCA) is a community-based, lay care navigation intervention serving individuals with advanced illness and their caregivers in communities of color. ACCA was founded in 2013 by five African American pastors in response to the unmet needs of individuals with advanced illness. Our Advanced Illness Care Program (AICP) was developed in collaboration with church leaders, congregants, health systems, academic partners, community groups, and national organizations specializing in serious illness. The program serves as a bridge between individual needs and health care delivery by addressing the social determinants of health that disproportionately affect communities of color experiencing advanced illness. ACCA serves individuals in five Northern California Counties (Alameda, Contra Costa, San Mateo, Santa Clara, and San Francisco), as well as Los Angeles County in Southern California. We place particular emphasis on bridging the gap between the health system and the community, addressing disparities, and promoting equity within the African American and Latinx communities. ACCA is a program of Public Health Institute (PHI). For 60 years, PHI has provided comprehensive fiscal sponsorship to health and public health programs making a difference around the world. PHI provides operational infrastructure so that ACCA can focus on its life-changing work. All employees of ACCA are employees of PHI.
