What are the most effective ways to prevent accidents when working at heights in a manufacturing environment?

Activity Hazard Analysis is the first step prior to start of any work at height activity as this will be the basis of what are the preventive measures and proper PPEs, tools and equipment that needs to be prepared and will be utilized in this task. Assessment should be conducted by a trained WAH individual and must be communicated within the team for the workers to understand it clearly and provide their input to better to the job safely. AHA must be reviewed and update as the work progresses. I have managed a job previously where we need to work at height and we have to submit and get approval of our WAH procedures prior to start of the work and it is mandatory that you should have your proper assessment prior to start of this task.

O simples fato de se informar que será um trabalho em altura já indica o perigo. O problema será a identificação dos riscos associados. Neste ponto cabe a união dos diversos setores que operam no local. Dando um exemplo: há anos atuava numa planta petroquímica onde um montador de andaime faleceu por queimadura. O que tem a ver montagem de andaime com queimadura (desconsiderando os efeitos do sol)? Nada; a princípio. Porém, no local, passava uma tubulação com vapor d'água que rompeu próximo à montagem do andaime. Isto caracteriza a necessidade de avaliação total dos eventos participantes, análise criteriosa, treinamentos bem conduzidos, acompanhamento das atividades.

In my experience, I agree that selecting the appropriate equipment should be considered to prevent WAH accidents, for example getting safety harnesses with fall arresters.

Fall arrester should be use while working in height above 2 meter. Make sure is he getting the perfect position to do the particular work. Monitor him all the time. And a lot.....

Base on my factual experience the first days of working in industrial environments are focusing on introducing of new ERA in different aspects not only organizational structures but also top important aspects like safety. It can be done by mentioning activity during probationary work period and deep training on specific working conditions. Day to day having 1-1 meeting with new joiners to help and support them and exchanging ideas. Defining how much areas need to work and defining proper safety tools to reach their objectives. For complex item needs to invite expert team members to work on it for hard to access areas and rare accessibility conditions. Deep dive on issue to avoid reoccurring them and sharing examples to team to cultivate it.

Train everyone the importance of safety. Be aware of the hazard , be aware of the cost and also be aware of other co workers

Promote a safety-first culture where workers feel empowered to speak up about unsafe conditions or practices. Regularly review and update safety protocols based on incident reports and best practices.

From my experience the regular training, feedback sessions during training and regular review of the plans based on the opinions of workers of the particular environment foster the building of safety culture in organization. So, for the safety systems to be effective it should be peoples centered.
