How do you measure and monitor the effectiveness of root cause analysis solutions?

I have seen many new products, programs and business goals fail because a clear goal/goals were never clarified and/or the goal was not aligned with customer needs/wants. Take the time needed to get this right. It is the foundation for all that follows.

It always pays off in the long run to continually ask "Why Because" until you exhaust all of the possibilities. Too often experienced individuals want to be a quarterback in the 4th quarter and jump to conclusions before gathering the facts. There are numerous analysis tools, however the key is to carefully and methodically determine the sequence of events "carefully" without contaminating the failure. It always pays to identify the lessons learned and perform a FMEA (Failure-Mode-Effects-Analysis) when developing a solution. A good risk mitigation plan always pays off.

Use Marked Feed Back Analysis or MFA or field data to consider in early new product designs or CA for field products to ensure optimum product integrity and oergormance.
