During a brainstorming session with the involved team members, and taking into account the available data, fishbone diagram and 5 whys can be used to identify root causes. It is important to accept feedback from others during this session and to dig deep to reach the root cause. If the root cause is not identified and resolved, the same problem will keep on recurring.

Also for them to work the relevant departments need to be fully engaged and participate as it is not only a QC function to identify and solve for these problems. My biggest challenge therefore, is that in my area of work there is never time for production, maintenance and other departments available to delve into this meaningful way of solving for issues. Each Problem/effect should have a single Fishbone diagram. 4.Using it in places involving logic and reasoning where decision trees or flowcharts may be more apt. 5.A Fishbone diagram developed for a problem may not be effective if the team size is too small for brainstorming or the team has limited knowledge regarding the process and issue. The causes identified may be limited and may not cover all possible causes.

Note: you can use the 3 legged 5 why analysis to identify the root causes related to systemic, Occurrences & non detection. Step 7: Once the team is concluded then go to the shop floor & validate the root causes.

Fish bone/cause&effect/ishikawa method/6M's method shall be used to solve complex problems whereas 5 why shall be used for simple issues. 6M's procedure involves verification of the 6 areas to understand where abnormalities occurred which shall be done with CFT's association. As per this procedure, the cause for the problem lies within the 6M's i.e., Men, Method, Material, Machine, Mother/Environment, Measurement. Investigating these 6M's will help us to identify the probable causes / Most likely causes along with the Root cause and addressing them will help us in further strengthening our process from upcoming failures. Why-why analysis will be used to interpret the simpler problems i.e., with one cause.
