Speedy escalation is key. Sometimes it may only ever be a hunch or may not have material evidence yet. The issue with taking your time to gather evidence and waiting for the right moment is that, If true - more damage is done as you consider your option and actions If false - it’s just an uncomfortable discussion, apology and retraction of your statement. There should be an escalation process in place that allows for whistle blowing or confidential escalation, so there is a more concerted business effort to verify the claim. The Space X methodology is unique to learn from. Carefully crafted unique message were sent out to suspected leaks. The message that was leaked out easily identified the perpetrators.
