A critical instrument for transition management is a risk and problem registry. It details the individual risks and concerns that will affect the transition, together with their assigned owner, description, impact, probability, status, and methods for mitigation and resolution. It can be kept up to date with a variety of instruments, including databases, spreadsheets, and specialist software, guaranteeing a systematic and successful approach to risk management.

One source of truth to record, monitor and track progress of risk and issues for any transition project is the RAID Log. It is recommended to host this on a Sharepoint with access to all the key stakeholders and participants of the project. (with audit trail/tracking)

This is a detailed report which usually is shared to stakeholders who can take action and help mitigate the immediate risks/issues impacting critical path of the project at a regular time interval. This report could also be a dump downloaded from the Sharepoint with specific filters (pre agreed) and shared by email to specific project stakeholders at time/mode agreed.

This log would include all the 'action items' which may not be completed by required timeline and may hamper progress of the transition project into 'Risk" log. If the 'predicted' risk comes true, (due to failure of an action that was unable to get completed ) then it moved to being an 'issue' and recorded in 'Issue log'. A detail explanation of risk/issue is key to the success of project. It should be assertive and well articulated. Regular and correction update is mandatory.
