In my experience, reach out to the client and shadow them in person if possible to see how their process is currently working to assess possible improvement areas. Talk with those involved in the processes on what works and what could be changed or even eliminated. If unable to do this in person, create virtual reviews as a less preferred approach. In either approach, make a few changes, monitor, get feedback and adjust. Then refresh the cycle as needed.

Ideally, jointly identify changes so there is buy-in and commitment vs some consultant or vendor suggesting what changes they should make. Then restart the data collection and assessment process that helped drive the team to this change point.

A lot of feedback processes tend to be digital based which helps collect data in a standard format and saves for analysis. Yet the key to this is will people even fill out the survey? Back to where I started the thread, if you can assess the processes in person and stay in engaged you will build rapport and get feedback along the way to get to the desired end game.
