When choosing your moderator, select someone who did not work on the project, but has knowledge of it. This will aid in preventing a biased lead session. The following format is what I use for the project and it can be used for post-mortem: What was the AIM: The goal was... Situation: What was the situation, define everything you can about it to gain a 360 view of what the current state prior to project. How Did it Work: Who, when, where, what, how, was the situation attacked. Results: What were the results, superficial, and then dig deep into the data to prove or disprove. Highlight: Highlight practices that should be repeated for future projects, and practices that did not add value, or took the work off course.
