Are you an Occupational Therapist with a passion for working to prevent falls and promote the well-being of residents living in care homes? From Oct 24 we will pilot a dedicated care home therapy service to help prevent falls in the 16 older person’s residential and nursing homes in Lambeth and Southwark. This presents an exciting and rare opportunity to establish and lead a small team based in these homes and contribute to the evaluation of the pilot.
You should have significant experience of delivering high quality; evidence-based rehabilitation and case management to frail older people and specifically with dementia to live well through identifying and addressing psychological needs and supporting meaningful occupation.
The success of this pilot will be contingent on your ability to form relationships and work collaboratively with care home staff to meet the therapy and falls prevention needs of residents.
You will be required to help facilitate effective working practices in the care homes by providing training, coaching and advice. You will need to demonstrate flexibility, creativity and the ability to support change in a fast-paced environment.
The team will be managed and supported by the Community Rehab and Falls Service (CRAFS). The role offers opportunities to develop leadership skills by enabling you to take a key role in the implementation and evaluation of this new care home service, in conjunction with partners from the NHS, primary care and social care.
