  • Prescribing Errors – Common Causes of Prescribing Errors From Clinical Correspondence. At NovaHS, we process over 250,000 documents each month, with clinical correspondence serving as a critical communication channel for prescribing recommendations across various settings. However, these documents also contribute to the risk of prescribing errors, which can have serious consequences. Identifying and understanding the common causes of such errors in clinical correspondence is crucial to preventing them and ensuring patient safety. Common causes which we have identified are: Poor communication – Variations in systems and incomplete or outdated records can result in prescribing recommendations based on inaccurate information, increasing the risk of errors. Illegible handwriting – Despite the use of typed letters, handwritten prescriptions still exist and can be difficult to decipher. Lack of standardisation – There is a variation in the layout of letters, and this creates difficulty in deducing what action is required or has already taken place. Delays in clinical correspondence – Communication is sometimes received a few months after the appointment, it may no longer align with the patient's current health status, potentially leading to outdated or irrelevant prescribing recommendations. High workload – High pressured environments and workload can lead to an oversight and recommendations have been associated to the incorrect patient.     I do love a good quality improvement (QI) project and appreciate the transformative power of a well-executed initiative. Here’s a structured approach that has worked well for us at NovaHS: Define clear objectives: Start with specific, measurable goals. What specific aspect of patient care are you aiming to improve? Clear objectives guide the focus of the QI project and help in evaluating its impact. Engage stakeholders: Involvement from all levels, from frontline staff to management, is crucial. Their insights not only enrich the project but also foster a sense of ownership and accountability, essential for sustainable change. Data-driven decisions: We base our strategies on solid data. We use both quantitative and qualitative data to understand the problem areas and to track improvements. This ensures any changes are evidence-based and targeted. Implement changes (gradually): Testing changes on a small scale before a full rollout. This pilot phase can help identify potential pitfalls and refine the process based on real-world feedback. Regular reviews and feedback loops: Set up continuous feedback mechanisms to monitor the effectiveness of the interventions. Regular reviews allow for timely adjustments and are essential for iterative improvement. Document and share learnings: Regardless of the outcome, documenting every step of the QI process is vital. We share successes and lessons learned within and beyond our organisation to promote best practices and collective learning. Quality improvement in healthcare is a dynamic and ongoing process. By systematically applying these principles, we have successfully delivered many meaningful changes, enhancing patient outcomes and service efficiency.
