Promising results in complex dementia care 📊 We are thrilled to share the remarkable outcomes from our pioneering Specialist Dementia Care Community (SDCC) at HC-One Meadow Bank Care Home in Preston. Our innovative approach, showcased by the ‘Sabrina’ community, aims to become a national model for dementia care, alleviating pressure on NHS and social care services. Key findings include: 📉90% decrease in distress-related incidents 📈60% of residents showing improvement in ‘Primary Behaviours of Concern’ 📈88% of residents improved their ‘Total Complexity of Need Profile’ Our SDCC model features Assistant Psychologists and Therapeutic Wellbeing Practitioners who create personalised care plans and engage residents in their favourite activities. Designed to feel like a ‘home from home,’ Sabrina includes therapy and sensory areas, activity rooms, and welcoming communal spaces for families. We are committed to expanding this successful approach, with our next SDCC opening in Scotland this summer. A really interesting read in The Guardian yesterday following a record number of people in England receiving a diagnosis of dementia over the past year. 📰⬇ Prof Graham Stokes, our Director of Dementia and Specialist Service Innovation at HC-One, shared his thoughts: “It is positive to see a steady recovery in the diagnosis rates in England, as this new data shows a slow but continued upward trend towards pre pandemic levels. “However, it is important to note that since the pandemic, there has also been a consistent decline in the proportion of people living with dementia who received a face-to-face care plan or review in the preceding 12 months. This can be problematic as without a review, an individual’s current needs are no longer known and may not be supported, which can cause a break down of care at home because of deteriorating health and an escalation in ‘behaviours of concern’, such as confusion, agitation or walking with risk. In turn, this often leads to an increase in the number of emergency and unplanned admissions to hospital that may have otherwise been prevented. “Also concerning is the fact that only 28.4% of those with a recorded diagnosis of dementia in September 2023 had their medication reviewed in the preceding 12 months, which again demonstrates that many people living with dementia may not have their current needs met. “To summarise, it is promising that we are seeing more people who are living with dementia receiving a diagnosis, but we still need to support those who remain undiagnosed (c.35%) as well as those who are diagnosed but without an active and accurate care plan. To put it simply, it is important to focus on diagnosis not being the end of a journey, but the beginning, by supporting people with appropriate and ongoing care and support beyond their diagnosis.” #dementiacare #diagnosis #dementia #hcone

Our innovative, therapeutic approach to dementia care aims to enhance support for individuals displaying distressed behaviour, moving away from traditional methods like anti-psychotic medication. Our SDCC model is unique, featuring Assistant Psychologists and Therapeutic Wellbeing Practitioners who create clinically evidence-based care plans and engage residents in meaningful activities. The environment is specifically designed for those with severe dementia, incorporating lower occupancy, enhanced lighting, and low arousal interior design. The success we've seen at our Meadow Bank care home SDCC in Preston, including a 90% decrease in distress-related incidents, inspires us as we bring this revolutionary approach to Scotland.
