High-quality, representative data is fundamental to the development and deployment of health systems. Research using – and technological innovations built on – health data can help to solve challenges from the diagnosis of rare diseases to more personalised care pathways. But what happens when this data does not accurately represent the experience of certain people and groups accessing care? And what does that mean for equitable access to health services? Our report explores these questions through the lens of gender. Our research found that NHS clinical information systems are not designed to accommodate gender diversity, conflating gender with physiological sex characteristics. Information about gender identity and sex is also recorded differently in different parts of the system. This could have a number of harmful knock-on effects for transgender and nonbinary people seeking healthcare. It could affect continuity of care. It could also mean they are not treated with dignity and respect. And ultimately, it could lead to invisibility of transgender and nonbinary people in health data. I recently had the opportunity to give a keynote speech to around 100 practical data leaders – CDOs and CTOs of organisations in the private and public sectors. Having recently started as the new Director of the Ada Lovelace Institute – an independent research institute with the mission to make AI and data work for people and society – I took the opportunity to look across the full spectrum of our work and to draw out the ways leaders in this space can help to make AI work for people and society. My basic premise was that we cannot be ambitious about data and AI without being thoughtful about it first. Our new report with Nuffield Council on Bioethics assesses the potential benefits, risks and appropriate role of AI-powered genomic health prediction (AIGHP) in the UK health system. While AIGHP could have some benefits within healthcare, there are significant risks concerning privacy and discrimination, as well as structural and societal problems that could emerge from its widespread roll-out for prevention at the population-level. The risks and concerns highlighted in the report relate to scientific uncertainty about the effectiveness of the emerging technology, privacy risks around the use of sensitive personal data on a mass scale, new forms of genetic discrimination (particularly in the context of health insurance), structural challenges surrounding NHS adoption of new technologies and a lack of evidence on public attitudes towards use of the technology. The development of advanced 'AI assistants' is seen as a logical next step for major technology companies, following significant investment in foundation models. There has been considerable buzz following announcements by companies like Google, Meta, Microsoft, Anthropic and OpenAI to create AI assistants that mediate the entire user experience of their products. Some of these AI assistants are being marketed as ‘smart interns,’ capable of carrying out any number of general and administrative tasks. Others have been developed for a specific purpose like providing mental health care, legal advice or companionship.
