This add-on could only be better if good items actually came by more than once in a blue moon, but you know, where's the fun without the challenge, right? The DLC adds lilith and keeper(yet to unlock) both of which add their own unique play to the game. Lilith has a little follower right away, that copies her tears, so yay, double shots? Nope, she's blindfolded. She gives you a great challenge as you have to change your gameplay a good bit, almost as much as that item no one wants, guillotine! but keeper, the triple shot without the decrease in max tears, runs off of coins instead of hearts. No problem right? You can only have two coins, each taking one hit no matter what floor, replenshed by coins, lowering your income, and suffering greatly if you try to take a devil deal. Alongside these two characters, the afterbirth gives a lot of great and hindering items and trinkets, such as the supermagnet which draws everything into you, from enemies to that one coin you couldn't reach before. Another thing this dlc brings about is the amazingly awesome greed mode. It adds a new boss, super greed, which can be tough without the right stats. You go through floors a bit differently, and don't have any other objective besides super greed, and adding coins to the greed machine. The greed machine drops after you kill supergreed, and has many valuable unlocks, such as holy mantle on the lost, and once you get 1000 coins in the machine, you unlock Keeper. Another few bosses are added as well, including Hush, the other tough nut to crack in this game, where you have to dodge the worst bullet hell yet, while trying to widdle down his high hp. What's worse is Hush has defence, that stuff that reduces damage done. Seems easy enough until you realize, you have to be pretty speedy and defeat it lives by 30 minutes in order to have the door to Hush's home, the blue womb, open. It also changes the way you unlock the lost. A way in which instead of finally unlocking the lost, I beat the lamb with like 5 items, while afk, getting ready for school.
I highly recommend this dlc if you've played Binding of Isaac before, or to buy it with the game itself, but to disable it for first time players. It's better to learn how to play on rebirth as this just makes it harder. Have fun with this dlc, and enjoy the pain that the players have to endure in order to in!

Really simple game, no money required for anything, simply a game to be a game, based off a fun game made by the same developers. Also one of the best clickers I've ever played.
