Missing annotations

I noticed that recently some of the annotations got cut off on some pages. (Pages: 10, 15, 22, 25, 29, 30, 38, 62, 73, 81)

Achievements not unlocking

Has anyone else had this issue? Got a few endings, but the respective achievements are still locked. Tried deleting my save and going through them again, but that didn"t work either.
