    “Dementia Singapore is stepping up to be the leading domain expert in providing community palliative care for people living with advanced dementia,” shared Jason Foo, CEO of Dementia Singapore. He was referring to plans to aid a targeted segment of the dementia community as part of Dementia Singapore’s latest Post Diagnostic Support (PDS) Plus programme, funded by Lien Foundation. He noted some of the current gaps in the system, from the limited support in the transition from daycare to home care for persons with advanced dementia, to the lack of engagement when planning for future needs and capability in management. But he also affirmed that while information and resources on palliative care are currently fragmented, “we are learning to make things better”. Dr Philip Yap, Senior Consultant, Department of Geriatric Medicine, Khoo Teck Puat Hospital and Chairman of Dementia Singapore, joined in to share how important palliative care is for people living with dementia. He stressed the need to provide for the person’s total needs and care, while upholding the person’s dignity and maintaining quality of life. The phrase Yap chose to emphasise is “person-centred care”, adding as well how communication is key to the goal of empowering those impacted with knowledge.

    A dementia diagnosis need not mean the end of life as you know it. Since 2022, our Care Beyond Walls project, in partnership with AWWA Ltd, has been breaking boundaries to design a new form of care for persons with dementia. Our approach focuses on personalised, interest-based activities and regular opportunities for our clients to venture out and be supported by the community.   During our recent visits to host organisations, we saw firsthand how our seniors are making meaningful contributions at SOSD, Kong Meng San Phor Kark See Monastery 光明山普觉禅寺, Elias Park Primary School, Sri Darma Muneeswaran Temple, and Willing Hearts Kitchen.   Our project aims to challenge conventional dementia day care services, enable persons with dementia to use their skills and cognitive reserves, and help them stay integrated in community life. We’re identifying and building supportive community partners, deepening Person-Centred Care practices, developing services for caregivers, and creating a dementia-friendly community around AWWA's existing Dementia Day Care Centre @ Ang Mo Kio.     Helping patients help themselves often means reconnecting them with feelings of amazement, wonder, and hope—emotions that can fade after prolonged illness. Non-pharmaceutical interventions like magic can reignite these feelings, significantly improving overall patient well-being. Even when patients know it's a magic trick, the feelings of hope and joy remain undiminished. The real magic lies in the effort and care behind each trick, creating a sense of connection and appreciation. This result is due to the dedication of healthcare professionals at Tan Tock Seng Hospital's Department of Psychology and Geriatrics Department. Their willingness to learn new skills to engage their patients demonstrates profound kindness, care, and support!     Breathlessness affects 60 - 90% of individuals with chronic heart or lung conditions. Without the right knowledge or skills, these individuals often dread physical activities as simple as showering - which can make them feel out of breath. Recently, with partners AWWA Ltd, Ren Ci Hospital and St Luke's ElderCare Ltd, we launched the Air Master cardiopulmonary rehabilitation service in collaboration with Tan Tock Seng Hospital to improve the quality of life for these individuals by helping them manage their breathlessness effectively.     Meet Singapore’s first lion dance troupe for seniors, including those wheelchair-bound. The 23-member troupe went through a training programme comprising a senior-friendly choreography developed by 184-year-old Chinese clan Kong Chow Wui Koon and heritage consultancy Bridging Generations It also featured a lion dance head with rewired pulleys and low-noise drum and cymbals redesigned by industrial designers at NextOfKin Creatives. The project is a continuation of the Lien Foundation’s experiment to strengthen the health and well-being of our ageing population by identifying untapped assets from the heritage, arts and sports sectors. Mr Lee Poh Wah, CEO, Lien Foundation, said: “Lion dance is synonymous with vitality, community, celebration and life - the very things we don’t associate with seniors. Seeing them perform such a dance thus helps us break down walls. The lion can be a Trojan horse to fight ageism.” With the completion of the pilot and our support, the team is looking for other partners to take it to the next phase.
