♦ Work permits are issued and hardly any spot audits happen to verify their adequacy or efficacy ♠ Trainings &TBTs are conducted and rarely quality of trainers or content is audited ♣ Harnesses & safety gadgets are worn at construction sites but no time to inspect for their fitness ♥ UA, UC, Near misses are reported but quality of reported data is not even on the agenda ♦ Staff is so busy generating reports and filling forms that hardly any quality time is left to invest in the field ♠ Manpower is slashed, budget is sliced but the goal of zero remains I can go on and on. Management of “Subtle” Changes Subtle Changes are those changes done in the field which does not lead to any change in any of the Engg Drawings.  Subtle changes in the field can lead to & have led to catastrophic events.  All changes (However small – not replacement of kind) receive appropriate review and approval.  PSM requirements for all such modification established/ documented & authorized.  Site to establish & implement written procedure to manage Subtle change to facility.  Communication to ensure understanding of; o What constitutes a change. o Difference between a technology change & subtle change. o System in place to manage each type of change
