What are the most effective strategies for promoting safety culture in an organization?
Many times the employees and ground staff involvement is not considered while planning or preparing the safety management system, their engagement is much required to ensure the effective safety management system. Also the cultural and behavioural aspects are key to make the management system effective. The employee engagement system is required at all levels be it planning execution or measurement of safety performances.

In my opinion the the vision, mission and target or baseline shall be well defined and implemented across the functions and objectives are clearly outlined. This could help the organisation to achieve safe or effective safety management systems.

How can you find reliable safety guidance for plant operations?
I believe in intrinsic safety that a device or machine should possess. And the device should not be working if these safeguards are bypassed. The team should be trained enough to make sure that the machine should not be modified or damaged in its OEM configuration in any condition. And procurement team should be aware and trained properly to consider or look into the safety aspects of the equipment and machines that are being purchased.

Apart from from right tool for right job, the focus should be the trading and awareness. And the integrity of the tool shouldn’t be compromised while working with the tools. In most of the cases the upkeep and maintenance of the tools are not done properly as per the their OEM requirements or standards.

Every unsafe Acts that you observe at your site, can be categorised while analysing based on human failure into the following Segments: 1. Errors and 2. Violations - Errors are unintended decisions and Violations are deliberate deviations. As per HSG 48, Errors can again be categorised into 1. Skill Based Errors & 2. Mistakes, Likewise Violations into 1. Routine, 2. Situational or 3. Exceptional. Why to do it? Guidance to Budding EHS professionals: In the early part of my career, donning the hat of "Responsible engineer" and "Site Responsible engineer ' (designations used then) most of the technical calculations had to be done by self without depending on a vendor to do it. If I had to install a tank, I had to show calculations of vent sizing or if I had to weld a nozzle to a tank, I had to show sizing of the pad plate (to relieve stress concentration), details of tell tale holes and weld details to my seniors. I still remember the day (year 2005) I got a dressing down by my senior when a ladder was built without a welder qualification and bad quality of welds. Even when we use vendors , they were questioned on the "basis" of calculation and standards used. The learning was much deeper. Now we are slowly moving to a "vendor-dependent" culture. The vendor decides and we don't ask questions on design calculations. We don't even review. This is leading to 1. Knowledge deficit as everything is outsourced to the vendor 2. Sub standard selection and installation since we don't know or have the knowledge to verify. Post is not to blame the vendor but to clarify our responsibility as EHS professionals in giving adequate information to the vendor and to ensure that vendor has chosen the right equipment. Discussions with the vendor is value addition because they are the subject matter expert, so each question to them will improve your knowledge, a notch higher. A few examples to qualify my concerns We should know for eg what MAWP, MEOP stands for and different kinds of other pressures in a pressure system. If we don't know this, we don't understand the nuances of pressure and settings of pressure relief systems. Fire extinguishers are bought with spec of just class and capacity. But what about spec on square feet coverage 5A-25B (an eg). Vendor will focus only on class and capacity because that was the only criteria asked for. Design of lifeline system without understanding the basis of capacity and how capacities are arrived and calculated. I often get a wrong answer that anchor points are designed for a person's weight safety margin when there are standards available for it . As safety professionals it is imperative that we get into details, understand the nuances of design, understand specs, read manuals and pick safety critical elements. If we don't do it , the vendor will choose what he feels is the best but won't be fit for purpose and can lead to failure. In safety, failure means accidents. Our role is to prevent it .
