    Our ancestors were super smart! They were deep engulfed into their roots, culture, nutrition & lifestyle.They had diverse & dense gut & oral microbiome No wonder, onset of chronic diseases such as type 2 diabetes, obesity, cardiovascular diseases, NAFLD, cancer were rare/non-existent. But the modern life that we see today is filled up western practices, making us develop chronic diseases at an accelerated pace. Specifically type 2 diabetes, cardiovascular diseases, cancer has become rampant. Specifically, we have understood dietary fats wrong. Still we have seen a lot of content & propaganda telling that saturated fats are bad. Even doctors would restrain you from consuming saturated fats. This should not be a surprise as doctors are never taught nutrition. Let me tell you the biochemistry behind how deficiency of Pentadecanoic acid ( C15:0), an odd chain saturated fatty acid, could lead to metabolic diseases such as Type 2 diabetes, fatty liver disease, anemia & more. Our cells have a cell membrane that has layers of lipids. One of the layers of lipids is fatty acids. Our cell needs healthy dietary fats - saturated, monounsaturated fats to maintain its structure & fluidity. Specifically, red blood cells need C15 to maintain its structure & function. When we do not consume much of C15, -It makes our cell structure fragile & weak, leading to lipid peroxidation.  Free iron in the cell combines with lipid peroxidation to produce ROS which causes mitochondria dysfunction. -Weak & fragile cells are eaten up by macrophages in the liver & are replaced by new red blood cells & iron gets back into circulation. However, when there is excess of this cellular fragility, corpses of iron get deposited in the liver, leading to iron overload & ferroptosis. This damages the liver & leads to inappropriate glucose response, leading to fatty liver disease & type 2 diabetes. Excess iron also goes to pancreas, impacting beta cell function & causing type 2 diabetes  - Consumption of seed oils & inflammatory Omega 6  instead of C15 could alter structure of cells & disrupt intracellular signaling. This interferes with production of nitric oxide. This beneficial gas is a signal for GLUT 4 to go to the cell membrane, binds to glucose & clear it off the circulation.     Cancer has been regarded as a genetic disease caused by genetic mutations driven by somatic mutation theory. You would be have influx of number of DNA testing or early cancer detection companies claiming they could detect risk/early cancer onset looking at certain genetic mutations, circulating tumor cells or physical examinations These all are driven by a financial model to make money out of personalized chemo/immuno or radiation therapies irrespective of success of such therapies throwing toxins inside our body. & then we have some new age VC funded startups setting up dialysis like chains to offer these toxin laden therapies. Unfortunately, none of these companies/healthcare institutions have a clear understanding of how cancer manifests & still treat the human body as black box. It has now been found that somatic mutation theory(SMT) is a flawed approach & cancer is a mitochondrial metabolic disease(MMT). In a recent research it was discovered there was absence of genes mutations & chromosomal abnormalities in 73 out of 210 cancer cases by sequencing their biological samples(1) while driver gene mutations, said to be responsible for a number of cancers, was found in normal cells which are non cancerous (2). It has also been found that there exists genomic heterogeneity in different tumor cells which itself makes these targeted therapies sub-optimal & genetic mutations can easily bypass them. So what causes Cancer ? It is damaged respiration( mitochondria dysfunction) followed by compensatory fermentation(4). Yes it is mitochondria dysfunction caused by chronic inflammation, carcinogenic toxins, viruses,  environmental toxins & more which shuts down the kill switch that controls cell cycle.     We all know that cancer has become the biggest epidemic mankind has ever faced with 104 Indians having lost their life to this dreaded disease in the year 2022. However, this is just the tip of iceberg It is expected that the number of people developing cancer would increase by 57% by the year 2040. This is shocking as we have all technologies & ecosystems in place to understand the mechanism of cancer & develop preventative solutions! The modern healthcare system is focussing on wrong targets- genetic mutations while the real cause is mitochondrial dysfunction & nuclear genetic mutation is downstream epiphenomenon of damaged respiration/oxidative phosphorylation insufficiency I have been seeing time & again, healthcare practitioners focus on prescribing chemotherapies, immunotherapies & other therapies with a hope to suppress genetic mutations.The success rate is dismal. There are number of reasons: -Reluctant to accept that cancer is a metabolic disease -Genomic Heterogeneity between primary & secondary tumor sites. -Genomic Heterogeneity among different organs -Lack of understanding of role of nutrition to boost the effectiveness of personalized therapies  -Failure to understand the molecular mechanisms underlying  metastasis  -No credible genetic mechanism to explain seed & soil hypothesis & fails to understand the preferred non-random organs as secondary tumor sites of metastatic cancer cells. Even the DNA testing companies would claim they can detect the risk of cancer by identifying your predisposition to certain cancer. For instance, these companies may tell you have BRCA1 gene & you have risk of breast cancer. But remember genes do not translate into function & underexpression or degradation of BRCA1 is the outcome of mitochondria damage. As such we have seen huge information asymmetry when it comes to understanding the molecular mechanism of cancer.     We often feel low on energy & tired specifically during the post lunch session & look to grab that cup of coffee for that quick energy boost! Many of us also consider coffee as a stimulant for brain health! No wonder, we love coffee Coffee offers lot of polyphenols which stimulate beneficial biochemical functions inside the body & protect against free radicals Coffee stimulates the production of hormones such as gastrin &  cholecystokinin that acts up digestive processes in the stomach & colon. In the stomach gastrin trigger release of gastric acid that promotes stomach contraction & hence promotes digestion In the colon, cholecystokinin increases the motility by causing smooth muscles of the colon to contract. Both these hormones help us poop. However, certain compounds in the coffee could trigger pro-inflammatory biochemical functions -Coffee is acidic with a pH level of 4.5 to 6.5. This could make our oral environment acidic which can become breeding ground for pathogens. -Coffee is a histamine liberator & those who have  gut microbiome derived high histamine production or histamine intolerance, coffee could lead to fatigue -Coffee has a low to moderate amount of oxalic acid.     For years, cancer has been regarded as a genetic disease with nuclear genetic mutations- overexpression of oncogenes & underexpression of tumor suppressing genes as the root cause. The genomic instability in the tumor cells is considered to primary reason for various cancer hallmarks including proliferative signaling, evasion of growth suppressors, resistance to cell death, replicative immortality, enhanced angiogenesis, and activation of metastasis It is important to understand that by telling you that cancer is a genetic disease, the current healthcare system makes you believe that only solution they have is these personalized therapies However, the fact is cancer is not a genetic disease but a mitochondria metabolic disease & is caused by mitochondria dysfunction( damaged oxidative phosphorylation). We all respire & get energy in the presence of oxygen when via aerobic respiration oxygen is converted into carbon dioxide & water. This is normal way of generating energy  However in case of damaged respiration, our cells lose this normal functioning & cannot use oxygen to produce ATP. Rather it uses it to produce Reactive oxidative species( ROS) which are carcinogenic & mutagenic. ROS damages DNA, RNA, lipid & protein causing genetic mutations Cancer cells can operate in a low oxygen environment & since the control switch( mitochondria) is gone, they start to grow & proliferate utilizing ancient fermentation pathways. These deadly cells ferment glucose & glutamine using glycolysis & glutaminolysis, producing lactic acid & succinic acid as a result. This generates an environment of acidification which further damages oxidative phosphorylation & provides a shield to cancer cells which protects them. We know that during cancer onset & progression, patients lose their appetite, leading to symptoms such as weight loss, loss of skeletal muscle & more. Considering this, your oncologist may tell you to consume a lot of glucose , sweets to gain weight. Probably, oncologists have an obscure understanding of nutrition. Consumption of glucose feeds cancer cells on one hand & makes these personalized therapies ineffective. Therefore a personalized nutritional therapeutic plays a key role to resolve cancer given that it is a metabolic disease.     Over the last few years, we have been hearing a lot about aging, how to slow down aging, reverse aging etc… A number of companies in India have mushroomed selling supplements such as NAD &/or offering DNA methylation tests to give you a picture about longevity If we go back around a decade ago, it was believed that telomere shortening & DNA methylation are the key factors that impact our aging & longevity. Thanks to research &  science, we now know that DNA methylation is just one of the factors for aging while telomere shortening can be inhibited. To have a healthy aging, we have to take care of our cellular health- ability of our cells to be healthy, to replicate & produce more cells. When our cells find it difficult to do these jobs(specifically when cells are fragile due to consumption of inflammatory oils & fats), it triggers a situation of aging, which by the way is another disease. The focus as such has to be on multiple aspects which impacts our cellular health & hence aging. -Pro-inflammatory activities/metabolites secreted by both gut & Oral microbiome -Epigenetically, changes in expression of human, microbial(gut, oral) genes -Mitochondrial biogenesis -Oxidative stress -Nitric oxide. Nitric oxide produced either by eNOS or nitrate-nitrite pathway plays a key role in longevity by -Upregulating telomerase activity & inhibiting telomere shortening, thereby delaying cellular senescence -Regulating & stimulating mitochondrial biogenesis -Suppressing inflammation Oxidative stress.     Human biochemistry is very very complex. We have 38 trillion cells & around 25000 human genes while our body is home to 2 to 20 million microbial genes. More than 50% of cells are microbial.There exists a symbiotic relationship between human host & microbes. We feed these microbes & in turn these microbes work in synchrony with us to keep us healthy. How do we feed these tiny friendly bugs ? Yes, the food we eat. Hippocrates once said : “Let food be thy medicine and medicine be thy food.” This is so true,  still it will takes decades to really understand nutrition as we fall for obnoxious advise of influencers, fitness experts, coaches & more Every third person we speak to would be more than willing to give you nutrition advice. This has literally become a business model .The more prominent advise we hear daily  “ Oh count your calories”, create a calorie deficit & everything would be fine” “ eat veggies”. True there are a lot of minerals & vitamins & veggies but do all veggies provide benefits for everyone ? Is cabbage good for you ? What about Spinach? This flawed advice is based on the assumption that everyone metabolizes food similarly which is wrong. Just think about it- you & your spouse eat the same meal for dinner but your bowel movements differ- It is clear that you & your spouse metabolize food differently. Food is more than fuel- it is information, it is a container for thousands of molecules, it tells your DNA what has to be done, and communicates with your cells. The food you eat is first available to your microbes. These microbes senses each of substrates/molecules & express a particular gene(DNA to RNA) that codes for a protein that perform a particular biochemical function be it metabolizing a food, absorbing nutrients, producing metabolites & more For instance Omega 3 communicates with you immune cells “ everything is fine, you can rest” Or your gut microbes may sense sulfate in cruciferous vegetables & tell immune cells “ Oh I am going to synthesis sulfate into H2S, please start doing inflammation “ Or even butyrate producing microbes may sense resistant starch & tell immune cells “not to worry I am producing butyrate that is going to strengthen intestinal barrier & also giving food for intestinal cells , so enjoy your feast” Unfortunately, people who care more about calories would say “ Oh resistant starch does offer calories, there is no much benefit of consuming resistant starch”.     Going through a recent report on Indian healthcare has made us wonder where is healthcare aka “sickcare” heading? -14% inflation in healthcare costs ->14% of hospital check in attributed to chronic kidney disease  - % of population visiting hospital for tumors is more than those visiting for heart ailments - 62% of hospital expenditure is paid out of pocket -Healthcare expenditure accounts of 7.9% of average household Now look at these concerning statistics > 10% population dealing with diseases & another 12% who have diabetes & are not aware > 10% having CKD > 13% have high risk of different forms of cancer  > In 2022, 2500 Indians lost their lives to cancer daily Obviously we are becoming more depressed, metabolically unhealthy, heavier, infertile every year. The more we spend on healthcare, the more our health is deteriorating. The more symptoms we have, the more drugs are being subscribed As individuals, what should be focused on ? 1)Meeting the monetary gap by subscribing to more insurance & medical loans or 2)spending money on things which could help us prevent these diseases from happening &/or cure them?  Obviously point no 2 Hospitals & current healthcare model is a financial model- they make money only when you are sick.They see you as a lifetime subscriber.     Since 2018, the healthcare sector in India has witnessed growing technological footprint to drive more deeper insights about an individual’s health &/or blend various post disease symptomatic conditions to make the symptoms more bearable. Some of such technologies adoption are restricted to wearables & non invasive models such as continuous glucose monitoring machines(CGM) that measure your glucose response to food. Needless to mention these are west copied models with not even a single iota of innovation. Why even after increased adoption of CGM, still the number of people developing type 2 diabetes is increasing daily? The inherent flaws have never been addressed & we this another which we assume is a solution to our metabolic health issues. Unfortunately, that is not the case as these devices never measures your molecular or cellular biochemistry -CGM just tells inflammatory response to a food, not to a food molecular/substrate -CGM does not measures the underlying cause of inflammation or various inflammatory pathways -Food is a combination of 100’s of substrates. How each substrates interacts with your biochemistry is what matters -Your food response is not static but changes every 1-2 months & therefore CGM may put you in confusion as to why a particular food which was good for your biochemistry 2 months back triggers inflammation. Probably because CGM does not measure your biochemistry & is nothing but a financial model & guess work However, the major issue which we miss out on is how typical nutrition choices & places we dine can insult our cells. Yes the fastest way to develop type 2 diabetes is to eat food made in inflammatory, seed or vegetable oils that alters your cell structure & fluidity. Your cells cannot sense what is going on & cannot stimulate production of nitric oxide. Besides, these oils react with oxygen to trigger oxidative stress which oxidizes tetrahydrobiopterin(BH4). BH4 is a co-factor for production of Nitric oxide. Oxidation of BH4 to BH2 disrupts nitric oxide production. We all know nitric oxide is a signal that tells protein- GLUT 4 to go to the cell membrane, bind to glucose, clear it from circulation & transport it to the cell for storage or energy. Without nitric oxide, there is no GLUT4 action which results in high glucose in circulation. As a result the pancreas senses it & releases more insulin. What happens next is a vicious cycle of hyperglycemia-hyperinsulinemia & type 2 diabetes.
