Upgrade tap and build against supported Node releases #258

This removes Node releases that are no longer supported according to: https://github.com/nodejs/Release/tree/24afd11681#nodejs-release-working-group
This upgrades tap to the current release.

This updates the CI pipeline to run against the currently supported Node releases
according to: https://github.com/nodejs/Release/tree/24afd11681#nodejs-release-working-group
This also upgrades the tap dependency to the latest release (16.3.7) and runs
npm audit fix.
