Bias becomes an organization culture from starting with one person, for all of the points mentioned here, you can adopt a top to bottom pyramid intervention or vice versa depending on how much bias has dominated the organization, most times, it is preferable to be a top to bottom approach as Bias is passed down and seldomly from bottom to top.
