Driver License/Identification Card Application is used for original credentials, converting credentials from other jurisdictions, renewing credentials and obtaining duplicate/replacement credentials.

Effective June 26, 2018, if eligible, when you complete the application, you will automatically be opted in to register to vote unless you select to decline.
