Please download and complete the Community Outreach Event Request Form below and e-mail it to [email protected].
To be considered, you must fill out all portions of Community Outreach Event Request Form and submit it at least one month in advance of the event date.
In the Subject line please write: Community Outreach Event Request, your organization’s name, and the event date. (Ex. Community Outreach Event Request_DC Health_Aug. 15, 2022)

If you are looking to request mobile vaccination services, please be advised that DC Health does not have a mobile vaccination van.
