The Rapid Assessment Zone (RAZ) at Aberdeen Hospital in New Glasgow is reducing patient wait times and improving emergency department (ED) flow. Stacy MacDonald, the emergency department’s first nurse practitioner, leads this initiative. MacDonald's expertise and 27 years of emergency and transport nursing experience are pivotal to the success of the RAZ. “It’s amazing to see the huge decrease in the number of patients who leave without being seen. This means patients get the care they need faster and allows me to diagnose serious conditions that may otherwise have gone undetected,” said MacDonald. Read the full story here:https://lnkd.in/d6mvS24g As part of the Emergency Care Plan announced in January 2023, the Rapid Assessment Zone (RAZ) model has been implemented in six emergency departments across the province, transforming the delivery of emergency care in Nova Scotia. 📷 Photo of nurse practitioner Stacy MacDonald

  • Empowering patients through collaboration: One Door Chronic Disease Management Centre’s relocation The recent relocation of the One Door Chronic Disease Management Centre in New Glasgow to a larger, more accommodating space marks a significant milestone in supporting those managing chronic diseases in Pictou County. “This relocation is a game-changer in our ability to offer comprehensive support to our patients,” said Katy MacDonald, Health Services Manager, Primary Care & Chronic Disease Management in Pictou County. “With this expansion, we can better address the diverse needs of our community with more accessible and tailored services.” While chronic diseases are manageable, they disrupt daily life and require continuous attention to alleviate symptoms and prevent complications. Recognizing these challenges, the new space fosters a collaborative, patient-centred approach, enabling seamless access to comprehensive care. The centre provides a workspace for a multidisciplinary team, including nurse practitioners, chronic disease management nurses, dietitians, social workers, respiratory therapists, and pharmacists, who collaborate to empower patients to navigate their health journey effectively. “Not only does the space encourage collaboration among team members, but having a designated space for group activities fosters a sense of community among patients, enhancing their overall well-being,” added MacDonald. Offering services under one roof eliminates the need for patients to juggle multiple appointments across various locations, streamlining their care journey and minimizing disruptions. The centre continues to redefine healthcare in Pictou County, ensuring comprehensive support for every patient from the moment they walk through the door.
