Rehab for Fall Prevention: Is Task-Oriented Training Enough? Traditional rehab often focuses on strength, stability and balance through task-oriented exercises. While effective in controlled settings, research shows these methods often fail in real-world environments. Daily life demands balancing in unpredictable scenarios, not just well-lit clinics. Despite structured rehab, fall rates in older adults remain unchanged (Clemson 2012). The issue? Sensory variability and multitasking—critical for real-life balance—are often overlooked (Teasdale 2002). Rehab heavily relies on visual cues, leading to a false sense of security and increased fall risk in less ideal situations (Peterka 2002). Additionally, overly task-oriented rehab may lead to maladaptive neuroplasticity (Grooms 2022), compromising balance when attention is divided. Sensory reweighting is key—rehab must train the brain to rely on proprioception and vestibular inputs, making balance more adaptable to real-life conditions. The exercise in this post’s video incorporates ECC training, dual-task challenges and sensory reweighting. Eccentric Training: Key to Fall Prevention The patient performs a Bulgarian split squat unassisted during the ECC contraction and uses arm support during the CON movement, making this an AEL exercise with ECC overload. ECC exercises are essential for addressing NM deficits in aging. These deficits impact the cortical, subcortical, and spinal levels of the CNS (Capin 2016). Impaired CNS excitability often limits effective muscle strengthening. ECC exercises bypass this inhibition and activate muscles more efficiently than CON or ISO exercises (Lepley 2015). ECC training targets fast-twitch fibers, disproportionately affected by sarcopenia and linked to increased fall risk in older adults (Keller 2019, Douglas 2017). Dual-Task Training: Addressing Attentional and Sensory Demands In the video, the single-leg strength exercise is combined with a working memory task. This promotes neuroplasticity and increases activation in the frontal lobe. Reduced frontal lobe activation is linked to impaired balance and higher fall risk (Mirelman 2017). Dual-task training reduces fall risk by combining cognitive and physical challenges, especially for older adults who experience more instability when multitasking (Shumway-Cook 2000). Sensory Reweighting The CNS processes vision, proprioception and vestibular inputs to maintain balance. Sensory reweighting ensures the most reliable inputs are prioritized (Kabbaligere 2016). Over-reliance on vision can increase fall risk when vision is compromised. The ECC exercise in the video integrates a perceptual task that challenges balance by engaging proprioceptive and vestibular inputs, reducing reliance on vision and enhancing adaptability to real-life, dynamic conditions (Peterka 2000). The Future of Fall Prevention To truly reduce falls, rehab must integrate eccentric training, dual-task exercises, and sensory reweighting.
