She's hurt because she thought they were reaching a point in their marriage where their coupling was not only for duty's sake, and feels even worse when she catches him pleasuring himself. She confronts him (or Ned realizes on his own that she's upset), and it turns out that Ned is just super paranoid about hurting her or the baby (he wasn't there when she was pregnant with Robb, after all) and he makes it up to her.
