In an alternate world where Hanahaki disease exists, you can get Hanahaki twice if you fall for the same person in a perceived unrequited love, even though it is scarce. The first time can be saved via surgery, but the only way to survive a second time is through proof of genuine requited love. The second time develops much more aggressively and faster, so usually, most people don't manage to do much before they fall under the disease.
And Taka, unfortunately, finds himself coughing out red petals despite the scar on his throat. -
