Summary
Sam must have scared him. He does that a lot. Guilt wells up in his chest, a sensation sickeningly familiar but no less uncomfortable because of it. He’d probably let go of Dean and tell him he can go back to bed if he didn’t feel like Dean’s presence was the only thing keeping him from shaking completely apart.

Summary
Dean's POV of Sam's lamp cord strangulation incident in S01e09, "Home."
An overwhelming, blinding light flashes throughout the whole house then, and Dean catches a glimpse of Sam’s pale features, washed stark white by the blaze. His thoughts turn to molasses as he sees Sam’s eyes are shut, his features lax. He’s not struggling with the cord anymore, not gasping for breath. It’s then that Dean drops to his knees and shields his face – whether from the light or from the sight of his brother, he couldn’t be sure.
You just got your brother killed, his head is screaming, and he can’t tell if the voice is their father’s or his own.

Summary
Dean doesn’t sleep that night, driving straight through. He can’t spare the time, and he wouldn’t be able to relax anyways. Even driving through the night, by the time the pair make it to Bobby’s, Sam is sweaty and shaking. Dean half carries him down to the panic room, hating every step. If there was any way to avoid this, make it easier for Sam, for Dean to take his place…
But his brother is going to have to suffer, and Dean is going to have to try to drink away the cavern that’s been ripped open in his chest. Because this time, he’s not leaving Sam.Series

Summary
Today’s hunt hadn’t even been a big deal. Famous last words, Sam knew. But his focus had been impaired; he’d been watching Dean like a hawk, terrified that the vamp was going to leap out of the dark and rip his brother away from him. As it turned out, Sam should’ve kept his own guard up.
“Sam. Sammy, you stay with me, dammit.” Dean is pressing hard on the stab wound, trying to stop the bleeding. The wound Sam was sure wouldn’t be a problem until they got back to the motel and he could stitch it himself.
Then he had collapsed, halfway to the Impala.
Sam thinks vaguely that he should be feeling sorry. He thinks he would, if he could focus on anything other than how Dean’s usual façade, that sense of calm he always fights for in life-or-death situations, is gone. Sam always sees through it anyways, but there’s something disconcerting about the raw fear in Dean’s eyes.Series

In the grand scheme of things, it shouldn’t matter how the kid died. The boys have a kidnapped reaper on their hands, plus Tessa showing up and trying to stick her hands into the whole mess.
But the moment he hears it was asthma, Dean is fighting the flashbacks, trying to focus on the case. On anything, besides the memories.
Five-year-old Sam fighting for breath out of nowhere; Dean screaming for John. Holding Sam’s hand, both of them clutching each other for dear life. Thanking a god he didn’t believe in even at nine, because he didn’t want to think about what would’ve happened if John had been on a hunt.
Eight-year-old Sam looking at Dean with panic written all over his face, because there’s suddenly no air to breathe. Dean grabbing the inhaler, which John had made Dean’s responsibility as soon as they got back from the hospital three years prior. Trying to take care of his brother even though he’s just a kid himself; trying to reassure Sam it will be okay even when he’s not sure it will.

For what seems like hours, the only sounds are Sam’s ribs splintering, and Dean’s ragged breathing. Then, when Dean finally starts talking to dull the sound of his brother’s bones cracking, he can’t seem to stop.
“Come on, Sam. You’re going to let a little water sprite take you out? After all the badass shit we’ve lived through?”
More cracking, more breaking, still no response.
“Sam, come on. You’ve got to breathe for me,” Dean pleads, even as he has to keep providing the breaths. Even as he’s the only reason why Sam’s getting oxygen at all.
Nothing. Sam’s skin is turning to ice and his lips are turning blue, as Dean’s tethers to sanity are quickly fraying.
“Please, Sammy,” Dean begs, voice breaking.
