- Notes
- ^ A municipality may have two or more incorporation dates for different municipal statuses throughout its history. If a municipality first incorporates as a village, then as a town, and most recently as a city (i.e., its current municipal status), then this column presents the municipality's incorporation date under its current municipal status.
